摘要
目的 探讨血清凝血因子Ⅲ、正五聚蛋白3(PTX-3)、纤维蛋白原(FIB)联合Geneva评分对肺癌合并肺血栓栓塞症(PTE)的诊断价值。方法 选取2020年1月至2022年1月青岛市胶州中心医院收治的52例肺癌合并PTE患者作为合并PTE组,另取同期收治的52例肺癌未合并PTE患者作为未合并PTE组。检测并比较两组血清凝血因子Ⅲ、PTX-3、FIB及Geneva评分,采用受试者工作特征曲线(ROC)曲线分析上述指标对肺癌合并PTE的诊断价值,采用单因素及多因素Logistic回归模型分析影响肺癌患者合并PTE的危险因素。结果 合并PTE组血清凝血因子Ⅲ、PTX-3、FIB及Geneva评分均高于未合并PTE组,差异有统计学意义(t=4.3780、8.432、11.075、12.555,P<0.05);单因素分析结果显示,病理类型、TNM分期、化疗、D-二聚体、血红蛋白与肺癌患者并发PTE有关(P<0.05);Logistic回归分析显示,血清凝血因子Ⅲ、PTX-3、FIB、Geneva评分、病理类型、TNM分期、化疗、D-二聚体、血红蛋白均为影响肺癌患者并发PTE的独立危险因素(P<0.05);由ROC曲线可知,血清凝血因子Ⅲ、PTX-3、FIB联合Geneva评分诊断肺癌并发PTE的AUC为0.914,高于四者单独诊断的0.748、0.753、0.745、0.763(P<0.05)。结论 肺癌并发PTE患者血清凝血因子Ⅲ、PTX-3、FIB水平呈升高趋势,且具有较高的Geneva评分,四者联合对肺癌并发PTE具有一定的诊断价值。
Objective To explore the diagnostic value of serum coagulation factorⅢ,pentraxin 3(PTX-3) and fibrinogen(FIB) combined with Geneva score for lung cancer combined with pulmonary thromboembolism(PTE). Methods A total of 52 patients with lung cancer and PTE admitted to Jiaozhou Central Hospital,Qingdao,were enrolled as the PTE group between January 2020 and January 2022,while other 52 lung cancer patients without PTE during the same period were enrolled as the non-PTE group. The serum coagulation factor Ⅲ,PTX-3,FIB and Geneva score between the two groups were compared. The diagnostic value of the above indicators for lung cancer combined with PTE was analyzed by ROC curves. The risk factors of PTE were analyzed by univariate and multivariate Logistic regression. Results The serum coagulation factorⅢ,PTX-3,FIB and Geneva score in the PTE group were higher than those in the non-PTE group(t=4.3780,8.432,11.075,12.555,P<0.05). The results of univariate analysis showed that pathological type,TNM staging,chemotherapy,D-dimer and hemoglobin were related to PTE in patients with lung cancer(P<0.05). Logistic regression analysis showed that serum coagulation factorⅢ,PTX-3,FIB,Geneva score,pathological type,TNM staging,chemotherapy,D-dimer and hemoglobin were independent risk factors for PTE(P<0.05). ROC curves analysis showed that the AUC of serum coagulation factor Ⅲ,PTX-3 and FIB combined with Geneva score in the diagnosis of lung cancer complicated with PTE was 0.914,which was higher than that of the four alone diagnosis(0.748,0.753,0.745,0.763)(P<0.05). Conclusion The serum levels of coagulation factor Ⅲ,PTX-3 and FIB in patients with lung cancer complicated with PTE tended to increase,and they had a higher Geneva score. The combination of the four has certain diagnostic value for lung cancer complicated with PTE.
