摘要
目的探讨托伐普坦联合复方丹参滴丸治疗急性心肌梗死并心力衰竭的临床疗效,以及对患者血清肌钙蛋白I(cTnI)、肌红蛋白(Myo)等的影响。方法选取医院2021年1月至2022年4月收治的急性心肌梗死并心力衰竭患者80例,按随机数字表法分为对照组与观察组,各40例。对照组患者采用托伐普坦治疗,观察组患者在此基础上加用复方丹参滴丸治疗。结果观察组患者的治疗总有效率为90.00%,显著高于对照组的72.50%(P<0.05)。治疗后,观察组的血清肌钙蛋白I(cTnI)、肌红蛋白(Myo)、血清肌酸激酶同工酶(CK-MB)、脑利尿钠肽(BNP)、心肌脂肪酸结合蛋白(H-FABP)水平均显著低于对照组(P<0.05);观察组左室射血分数(LVEF)和心输出量(CO)均显著高于对照组,但左房容积指数(LAVI)和舒张早期二尖瓣处血流速度/代表舒张早期二尖瓣环的运动速度(E/e')均显著低于对照组(P<0.05)。结论托伐普坦联合复方丹参滴丸治疗急性心肌梗死并心力衰竭,能有效改善患者的血清cTnI,Myo等水平,促进其心脏功能恢复。
Objective To investigate the clinical efficacy of tolvaptan combined with Compound Danshen Dropping Pills in the treatment of acute myocardial infarction complicated with heart failure and its effect on serum troponin I(cTnI)and myoglobin(Myo).Methods A total of 80 patients with acute myocardial infarction complicated with heart failure admitted to the hospital from January 2021 to April 2022 were selected and divided into control group and observation group according to the random number table method,with 40 cases in each group.The patients in the control group were treated with tolvaptan,on this basis,the patients in the observation group were treated with Compound Danshen Dropping Pills.Results The total effective rate in the observation group was 90.00%,which was significantly higher than 72.50%in the control group(P<0.05).After treatment,the levels of serum troponin I(cTnI),myoglobin(Myo),serum creatine kinase isoenzyme(CK-MB),brain natriuretic peptide(BNP)and myocardial fatty acid binding protein(H-FABP)in the observation group were significantly lower than those in the control group(P<0.05).The left ventricular ejection fraction(LVEF)and cardiac output(CO)in the observation group were significantly higher than those in the control group,while the left atrial volume index(LAVI)and the blood flow velocity at the mitral valve in early diastole/the motion velocity of the mitral valve ring in early diastole(E/e')in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Tolvaptan combined with Compound Danshen Dropping Pills can effectively improve the levels of serum cTnI and Myo and promote the recovery of cardiac function in patients with acute myocardial infarction complicated with heart failure.
