摘要
目的使用Meta分析的方法评价当归四逆汤治疗子宫内膜异位症(EM)的安全性和临床疗效。方法检索Pubmed、Web of Science、SinoMed、中国期刊全文数据库(CNKI)、万方知识数据服务平台、维普期刊数据库关于当归四逆汤治疗EMT的文献,检索时间自建库到2021年4月,选择试验组为当归四逆汤加减、对照组为单用西药口服的临床随机对照试验,依据本研究的纳入及排除标准,由2位研究者总计筛选出文献88篇,最终纳入8篇文献,总病例数为749例进行研究,其中试验组383例,对照组366例,运用Revman 5.3软件将纳入的文献进行质量评价和数据分析。结果最终纳入8项研究进行Meta分析,结果显示试验组总有效率高于对照组[RR=1.18,95%CI(1.12,1.26),P<0.00001],不良反应发生率低于对照组[RR=0.20,95%CI(0.12,0.32),P<0.00001],并且可显著降低血清炎症因子IL-8水平[MD=-19.03,95%CI(-22.94,-15.11),P<0.00001]、血清雌二醇和黄体酮水平雌二醇[MD=-29.02,95%CI(-35.67,-22.37),P<0.00001]、黄体酮[MD=-5.43,95%CI(-8.05,-2.82),P<0.0001],血清CA125水平[MD=-8.78,95%CI(-10.63,-6.92),总效应检验Z=9.26,P<0.00001],差异有统计学意义(P<0.05)。结论当归四逆汤加减治疗EM的临床疗效较单用西药口服更优,可提高治疗的总有效率,降低不良反应发生率,并且在降低血清炎症因子IL-8水平和血清雌二醇及黄体酮水平上更是优于激素类药物。
Objective To evaluate safety and the efficacy of Danggui Sini Decoction in Meta analysis on endometriosis(EM).Methods The literature of EMT in Pubmed、Web of Sience、SinoMed、CNKI,Wanfang and VIP were retrieved.The retrieval time was from the establishment of the database to April 2021.The experimental group was selected as the clinical randomized controlled trial of Danggui Sini Decoction,and the control group was selected as the clinical randomized controlled trial of oral administration of Western medicine alone.According to the inclusion and exclusion criteria of this study,a total of 88 literatures were screened out by the two researchers,and finally 8 literatures were included.The total number of cases was 749,including 383 in the experimental group and 366 in the control group,Use revman 5.3 software to evaluate the quality and analyze the data of the included documents.Results Finally,eight studies were included for meta-analysis.The results showed that the total effective rate of the test group was higher than that of the control group[RR=1.18,95%CI(1.12,1.26),P<0.00001],the incidence of adverse reactions was lower than that of the control group[RR=0.20,95%CI(0.12,0.32),P<0.00001],and the level of serum inflammatory factor IL-8 was significantly reduced[MD=-19.03,95%CI(-22.94,-15.11),P<0.00001],Serum estradiol and progesterone levels estradiol[MD=-29.02,95%CI(-35.67,-22.37),P<0.0001],progesterone[MD=-5.43,95%CI(-8.05,-2.82),P<0.00001],The differences are statistically significant(P<0.05).Conclusion The clinical efficacy of Danggui Sini Decoction in the treatment of EM is better than that of Western medicine alone.It can improve the total effective rate of treatment,reduce the incidence of adverse reactions,and is better than hormone drugs in reducing the levels of serum inflammatory factor IL-8 and serum estradiol and progesterone.
