摘要
目的:分析抗菌药物所致药品不良反应(ADR)发生的特点及规律,为临床合理使用抗菌药物提供参考。方法:收集本院 2016年1月~2020年12月上报国家药品不良反应监测系统的抗菌药物ADR 报告,并进行帕累托图分析。分析发生抗菌药物ADR的药品种类、累及器官的主要和次要因素及抗菌药物ADR首要种类所累及器官的主要因素和次要因素。结果:共收集抗菌药物ADR报告232例,女性123例(52.99%),男性109例(47.01%);护士上报208 例(89.66%),药师上报16例(6.89%),助产士上报8例(3.45%)。帕累托图分析结果显示,导致ADR的主要药品种类为喹诺酮类、头孢菌素及大环内酯类;抗菌药物ADR主要累及皮肤及其附件、胃肠系统;ADR首要种类喹诺酮类主要累及用药部位、皮肤及其附件。结论:抗菌药物临床应用要全面关注ADR,重点监测喹诺酮类、头孢菌素及大环内酯类;发生抗菌药物ADR时主要关注皮肤及其附件、胃肠系统,发生喹诺酮类ADR时重点关注用药部位、皮肤及其附件。
Objective To analyze the regularity and characteristics of adverse drug reactions (ADR) caused by antibiotics in our hospital,and to provide references for rational use of antibiotics.Methods A total of 232 antimicrobial ADR reports were collected from our hospital from January 2016 to December 2020,and Pareto diagram was charted.The antimicrobial drug involvement and organs/systems affected were categorized,and the major and minor factors of the organs/systems involved in the most ADRs types of antibiotics were analyzed.Results A total of 232 ADR case caused by antibiotics were collected,including 123 females (52.99%) and 109 male (47.01%);208 cases (89.66%) were reported by nurses,16 cases (6.89%) by pharmacists and 8 cases (3.45%) by midwives.Pareto chart showed that ADRs were mainly related to quinolones,cephalosporins and macrolides;The major clinical manifestations were related to skin and its accessories,gastrointestinal system;the most ADR types of antibiotics is quinolones,which mainly related to medication site,skin and its accessories.Conclusion We should take full account of ADRs of antimicrobial drugs with special focus on the quinolones,cephalosporins and macrolides.In case of adverse reactions of antibiotics,we should take mainly focus on the skin and its accessories and gastrointestinal system.In case of quinolone ADR,we should take focus on the medication site,skin and its accessories.
