浮针联合颌枕牵引治疗神经根型颈椎病患者的临床研究 被引量:11

Clinical Study of Floating Needle combined with Maxilloception Traction in the treatment of Cervical Spondylotic Radiculopathy

摘要 目的:探究浮针联合颌枕牵引治疗CSR患者的颈部活动功能以及对基底动脉血流动力学的影响。方法:纳入六安市中医院2019年1月~2020年12月收治的CSR患者126例,随机分为两组,其中研究组63例(颌枕牵引+浮针),对照组63例(颌枕牵引治疗),对比分析两组颈椎病临床评价量表得分,治疗前后VAS评分、CASCS评分和神经功能缺损程度评分量表(CSS)评分、超声检测基底动脉血流动力学变化(包括平均血流速度(TMFV)和阻力指数(RI))以及颈椎活动度等指标变化综合分析浮针联合颌枕牵引对CSR患者颈部活动功能及基底动脉血流动力学指标的影响。结果:治疗后,两组患者CSS评分、CASCS评分、VAS评分以及颈椎旋转、屈伸功能,血流动力学变化均较同组治疗前有不同程度的改善,治疗后复查CSS评分以及VAS评分发现,研究组均较对照组降低,而治疗后对照组颈椎CASCS评分比研究组明显降低;研究组在治疗后颈椎旋转及屈伸功能均较对照组显著增大;治疗后复查多普勒超声后发现,对照组的基底动脉血流动力学指标RI明显高于研究组,而TMFV明显低于研究组低;对照组总体临床有效率明显低于研究组。结论:浮针联合颌枕牵引能显著改善神经根型颈椎病患者颈部运动功能,缓解疼痛症状,消除局部神经水肿,改善基底动脉血流动力学,从而提高治疗CSR患者的总体临床疗效。 Objective To explore the clinical effect of floating needle combined with maxilloception traction in the treatment of cervical spondylotic radiculopathy and its effect on the blood flow dynamics of the basal artery.Methods From January 2019 to December 2020,126 patients with radiculopathy of cervical spondylosis admitted to Lu’an City Hospital of Traditional Chinese Medicine were included and randomly divided into two groups,including 63 patients in the research group (maxillocival traction + floating needle) and 63 patients in the control group (maxillocival traction).The clinical evaluation scale scores of cervical spondylopathy in the two groups were compared and analyzed.VAS scores before and after the treatment,CASCS scores and nerve function defect degree assessment scale (CSS) score,ultrasonic testing,basal arterial blood flow dynamic changes (including the average blood flow velocity (TMFV) and resistance index (RI)) and the change of cervical activity and other indexes such as comprehensive analysis of floating needle joint jaws pillow traction cervical spine movement function in patients with CSR and basal artery influence of hemodynamic indexes.Results After treatment,CSS score,CASCS score,VAS score,cervical rotation,flexion and extension function,and hemodynamic changes in 2 groups were improved to varying degrees compared with before treatment.The CSS score and VAS score of the study group were lower than those of the control group after treatment,while the cervical CASCS score of the control group was lower than that of the study group after treatment.The cervical rotation,flexion and extension function in the study group were significantly increased after treatment compared with the control group.The hemodynamic index RI of the control group after treatment was significantly higher than that of the study group,while TMFV was more lower than the research group;the overall clinical effective rate of the control group was significantly lower than that of the study group.Study group.Conclusion Floating needle combined with jaw-occipital traction can significantly improve the neck motor function,relieve pain symptoms,eliminate local nerve edema,and improve the blood flow dynamics of the basal artery in patients with radiculopathy,thus improving the overall clinical efficacy of CSR patients.
