摘要
目的探讨复荣通脉方联合前列地尔注射液治疗糖尿病性下肢动脉硬化闭塞症临床疗效。方法选取2018年1月—2020年1月期间北京中医医院顺义医院收治的糖尿病性下肢动脉硬化闭塞症患者120例,采用随机信封法分为对照组和观察组,每组各60例。对照组给予前列地尔注射液治疗,在此基础上观察组加用复荣通脉方治疗。治疗1个疗程后,观察比较两组患者临床疗效、中医症状积分、血液流变学指标及患肢血流情况。结果治疗后观察组总有效率为96.67%(58/60),与对照组总有效率85.00%(51/60)比较,差异有统计学意义(P<0.05)。两组患者治疗后中医症状积分均较治疗前明显降低,差异有统计学意义(P<0.05);且观察组中医症状积分较对照组明显降低,差异有统计学意义(P<0.05)。两组患者治疗后足背动脉血管内径、足背动脉峰值流速、足背动脉血流量及ABI均较治疗前明显升高差异有统计学意义(P<0.05);且观察组上述指标均明显高于对照组,差异有统计学意义(P<0.05)。两组患者治疗后全血低切黏度、全血高切黏度、纤维蛋白原、红细胞压积均较治疗前明显降低,差异有统计学意义(P<0.05);且观察组血液流变学指标均明显低于对照组,差异有统计学意义(P<0.05)。结论复荣通脉方联合前列地尔注射液治疗DLASO的效果显著,能明显改善患者临床症状,加快下肢动脉血液流速、抑制血栓的形成。
Objective To investigate the clinical effect of FurongTongmai Formula combined with alprostadil injection on the treatment of diabetic lower extremity arteriosclerosis occlusive disease.Methods A total of 120 patients with diabetic lower extremity arteriosclerosis occlusive disease admitted to the Shunyi Hospital,Beijing Hospital Traditional Chinese Medicine from January 2018 to January 2020 were selected and divided into a control group and an observation group by random envelope method,with 60 cases in each group.The control group was treated with alprostadil injection.Based on the treatment of the control group,the observation group was additionally givenFurongTongmaiFormula.The clinical efficacy,traditional Chinese medicine(TCM)symptom scores,hemorheology indexes,and blood flow of affected limb of the patients in the two groups were compared.Results After treatment,the total effective rate of the observation group was 96.67%(58/60),and that of the control group was 85.00%(51/60)(P<0.05).After treatment,the TCM symptom scores of the two groups were significantly lower than those before treatment(P<0.05),and the TCM symptom score of the observation group was significantly decreased(P<0.05).After treatment,the inner diameter,peak velocity,blood flow,and ankle brachial index(ABI)of the dorsalis pedis artery of patients in the two groups were significantly improved(P<0.05),andthe above indexes of the observation group were higher than those of the control group(P<0.05).After treatment,the whole blood low shear viscosity,whole blood high shear viscosity,fibrinogen,and hematocrit of patients in the two groups were significantly decreased(P<0.05).The hemorheological indexes in the observation group were significantly lower than those in the control group(P<0.05).Conclusion FurongTongmai Formula combined with alprostadil injection has a significant effect on the treatment of diabetic lower extremity arteriosclerosis occlusive disease,which significantly improves the clinical symptoms of patients,accelerates the blood flow rate of lower limb arteries,and inhibits the formation of thrombosis.
