摘要
心肌缺血再灌注(MI/R)损伤是指缺血心肌恢复血液灌注后,心功能不改善反而出现即刻恶化的现象。MI/R损伤是急性心肌梗死不可避免的危险事件,其致病机制尚不完全清楚。研究显示,导致MI/R损伤的病理生理机制可能与氧化应激、铁负荷增加和脂质过氧化增加等有关。铁稳态失调、脂质过氧化或谷胱甘肽代谢紊乱在MI/R损伤中起作用。而2012年提出铁依赖性脂质过氧化驱动的细胞死亡即铁死亡概念之后,学者们重新审视铁稳态失调、脂质过氧化或谷胱甘肽代谢紊乱在MI/R损伤中的作用,从而提出了铁死亡参与MI/R损伤发病机制。同时研究表明,阻断铁死亡可降低MI/R损伤的严重程度,并发现铁死亡抑制剂可有效修复I/R诱导的心肌细胞损伤,而我国传统中药在抑制铁死亡和治疗MI/R损伤方面取得了一定进展。本文综述了MI/R损伤发病机制中涉及铁死亡机制的最新研究进展,重点介绍靶向抑制铁死亡的中药,包括白藜芦醇、黄芩苷、矢车菊素-3-O-葡萄糖苷、柚皮素和醋酸棉酚等,为临床治疗MI/R新靶点的研究提供方向。
Myocardial ischemia/reperfusion(MI/R)injury is a condition wherein cardiac function does not improve but worsens immediately after perfusion is restored to the ischemic myocardium.MI/R injury is an unavoidable risk event for acute myocardial infarction.The pathogenic mechanism of MI/R injury is not fully understood.The pathophysiological mechanisms leading to MI/R injury are associated with oxidative stress,iron overload,and increased lipid peroxidation,among others.Ferroptosis,a newly identified iron-dependent lipid peroxidation-driven cell death,was coined in 2012.Since then,scholars have reexamined the role of iron homeostasis,lipid peroxidation or oxidative stress in MI/R injury,and speculated that ferroptosis participates in the pathogenesis of MI/R injury.At the same time,there is increasing evidence that some novel compounds targeting ferroptosis can relieve MI/R injury-related pathological symptoms in cell and animal MI/R injury models.It has been shown that inhibition through Chinese traditional medicine can attenuate ferroptosis,thereby alleviating MI/R injury.This review summa⁃rizes the latest research progress in the role of ferroptosis in MI/R injury by focusing on the potential treatment strategies targeting ferroptosis by traditional Chinese medicine,including resveratrol,baicalin,cyanidin-3-glucoside,naringenin and gossypol acetic acid.
