口服给药片剂精准分包方法的比较研究

摘要 目的比较不同口服给药片剂分包方法的剂量精确性,确保患者临床口服用药合理性及安全性。方法随机抽取南京医科大学第一附属医院及江苏省人民医院妇幼分院2019年3—6月住院药房共290698条片剂医嘱数据,应用不同分包方法对分包频率较高且药效受剂量影响较大的11种分剂量药片称重。依据9.0版《欧洲药典》中分剂量药片评价标准进行重量达标评价。结果1/2片分包方法中分药器法有6种药品达标,刀片切割法有5种药品达标,剪刀法有3种药品达标,手工掰分法有3种药品达标;1/4片分包方法均未达合格标准。结论无刻痕片1/2片分包时优选分药器法进行分包,刻痕片1/2片分包时优选刀片切割法和分药器法进行分包,分剂量片数不超过1/4片时宜选用磨粉分包法进行分包。 Objective To compare the dose accuracy of different oral tablet packaging methods,and to ensure the rationality and safety of clinical oral medication in patients.Methods A total of 290698 tablet doctor orders were randomly selected from the First Affiliated Hospital of Nanjing Medical University and the Maternal and Child Branch of Jiangsu Provincial People's Hospital from March to June in 2019.11 kinds of sub-dose tablets with high sub-package frequency and greater effect of drug effect by dose were weighed by different sub-package methods.The weight compliance evaluation was carried out according to the evaluation standard of divided dose tablets in the European Pharmacopoeia 9.0.Results In the 1/2 tablet packaging method,6 drugs were up to the standard by the dispenser method,5 by the blade cutting method,3 by the scissors method,and 3 by the manual breaking method;1/4 tablet none of the subcontracting methods met the eligibility criteria.Conclusion When sub-packing 1/2 non-scored tablets,the drug dispenser method is preferred for sub-packaging,and when 1/2 scored tablets are sub-packaged,the blade cutting method and the drug dispenser method are preferred for sub-package,when the number of sub-dose tablets does not exceed 1/4,the grinding powder sub-package method should be used for sub-package.
