摘要
目的分析重症肺炎特点及患者治疗要点,评价在抗感染机械通气治疗的基础上辅助中药方清金化痰汤的治疗效果。方法选择辽宁省人民医院2019年5月—2020年7月收治的80例重症肺炎患者,经医院伦理委员会批准、参与患者与家属知情同意。对照组患者采取抗感染机械通气治疗,观察组患者辅助清金化痰汤治疗,比较两组重症肺炎患者的整体治疗情况。结果治疗后观察组临床总有效率为97.50%,对照组临床总有效率为82.50%,两组比较差异有统计学意义,χ^(2)=5.0000,P<0.05;两组患者用力肺活量(forced vital capacity,FVC)、1 s用力呼气容积(forced expiratory volume in one second,FEV1)、呼气流量峰值(peak expiratory flow,PEF)等肺功能指标以及C反应蛋白(C-reaction protein,CRP)、白细胞介素-6(interleukin-6,IL-6)、白细胞介素-2(interleukin-2,IL-2)等炎症指标水平较治疗前改善,观察组较对照组改善明显(均P<0.05);治疗后两组患者健康调查简表(The MOS Item Short from Health Survey,SF-36)评分较治疗前明显改善,观察组较对照组改善明显(均P<0.05)。结论清金化痰汤辅助抗感染机械通气治疗重症肺炎预后效果理想,促进患者肺功能、炎症水平改善,进而提升患者的生活质量。
Objective To analyze the characteristics of severe pneumonia and the main points of treatment for patients,and to evaluate the effect of auxiliary Chinese medicine Qingjin Huatan Decoction(清金化痰汤)on the basis of anti-infective mechanical ventilation therapy.Methods 80 patients with severe pneumonia who were admitted to our hospital from May 2019 to July 2020 were selected,approved by the hospital ethics committee,and informed consent of the participating patients and their families.Patients in the control group were treated with anti-infective mechanical ventilation,and patients in the observation group were treated with Qingjin Huatan Decoction to compare the overall treatment of the two groups with severe pneumonia.Results Comparison of clinical efficacy between the groups,the observation group and the control group have a significant difference in total clinical effectiveness(97.50%vs 82.50%),χ^(2)=5.0000,P<0.05;forced vital capacity(FVC),forced expiratory volume in one second(FEV1),peak expiratory flow(PEF)pulmonary function,C-reaction protein(CRP),interleukin-6(IL-6),interleukin-2(IL-2)inflammation index levels were improved compared to before treatment,the observation group was significantly improved compared to the control group,all P<0.05;after treatment two groups’the MOS item short from health survey(SF-36)scores were higher than before treatment,the observation group improved significantly compared with the control group(all P<0.05).Conclusion Qingjin Huatan Decoction assisted anti-infective mechanical ventilation in the treatment of severe pneumonia has an ideal prognostic effect,promotes the improvement of patients’lung function and inflammation,and improves the quality of life of patients.
