摘要
目的分析整体式护理在行人工肝血浆置换治疗的肝功能衰竭患者中的应用效果。方法回顾性分析2018年1月至2020年10月于清远市人民医院行人工肝血浆置换治疗的80例肝功能衰竭患者的临床资料,其中2018年1月至2019年5月期间按方便抽样法随机抽取的40例患者设为对照组,给予常规护理;2019年6月至2020年10月期间按方便抽样法随机抽取的40例患者设为观察组,给予整体式护理。比较2组患者的肝功能指标、不良事件发生率及生活质量。结果干预2周和4周后,2组的血清总胆红素、丙氨酸氨基转移酶、天门冬氨酸氨基转移酶水平均较干预前降低(P均<0.05),且观察组均低于同期对照组(P均<0.05)。观察组血肿、感染、管路凝血以及管路和分离器堵塞的总发生率为10.00%,低于对照组的30.00%(P<0.05)。干预4周后,2组的生活质量综合评定问卷-74(躯体功能、心理功能、社会功能和物质生活状态)评分均高于干预前(P均<0.05),且观察组均较对照组更高(P均<0.05)。结论整体式护理可以改善行人工肝血浆置换治疗的肝功能衰竭患者的肝功能,提高其生活质量,以及降低不良事件发生率。
Objective To analyze the application effect of holistic nursing in the liver failure patients treated with artificial liver plasma exchange.Methods The clinical data of 80 liver failure patients treated with artificial liver plasma exchange in Qingyuan People's Hospital from January 2018 to October 2020 were retrospectively analyzed.The 40 patients randomly selected by convenience sampling from January 2018 to May 2019 were set as the control group and given routine nursing,and the 40 patients randomly selected by convenience sampling from June 2019 to October 2020 were set as the observation group and given holistic nursing.The indicators of liver function,incidence of adverse events,and quality of life were compared between the two groups.Results After 2 and 4 weeks of intervention,the levels of serum total bilirubin,alanine aminotransferase,and aspartate aminotransferase in the two groups were lower than those before the intervention(all P<0.05),and the levels in the observation group were lower than those in the control group(all P<0.05).The total incidence of hematoma,infection,pipeline coagulation,and pipeline and separator blockage in the observation group was 10.00%,which was lower than 30.00%in the control group(P<0.05).After 4 weeks of intervention,the scores of the Generic Quality of Life Inventory-74(physical function,psychological function,social function,and material life state)in the two groups were higher than those before the intervention(all P<0.05),and the scores in the observation group were higher than those in the control group(all P<0.05).Conclusion Holistic nursing can improve the liver function and quality of life of liver failure patients treated with artificial liver plasma exchange,and reduce the incidence of adverse events.
