摘要
目的探讨降钙素原和C反应蛋白(CRP)在新型冠状病毒肺炎(COVID-19)病人血清中的表达及其预后评估价值。方法选取2020年2—3月四川绵阳四〇四医院收治的119例COVID-19病人进行研究,回顾性分析病人临床资料,血清降钙素原、CRP水平及预后生存状况,将临床分型为轻型、普通型的病人列为轻度组(93例),重型、危重型的病人列为重度组(26例),依据病人住院30 d预后生存状况分为存活组(112例)、死亡组(7例)。采用免疫放射法检测血清降钙素原水平,采用散射比浊法检测血清CRP水平,并进行组间比较,采用logistic回归模型分析影响COVID-19病人预后因素。结果轻度组与重度组相比,性别、发热、咳嗽、腹泻、乏力、头痛均差异无统计学意义(P>0.05),与轻度组相比,重度组年龄、高血压、糖尿病、冠心病、慢性阻塞性肺疾病、呼吸困难比例、血清降钙素原[(0.52±0.11)μg/L比(0.13±0.03)μg/L]、CRP水平[(62.53±13.26)mg/L比(15.91±2.79)mg/L]均较高(P<0.05);与存活组相比,死亡组血清降钙素原[1.01(0.95,1.12)μg/L比0.17(0.14,0.20)μg/L]、CRP水平[96.27(89.94,96.27)mg/L比21.96(17.14,26.51)mg/L]均较高(P<0.05);logistic回归模型分析结果显示,年龄≥60岁、合并基础疾病、呼吸困难、高水平降钙素原、高水平CRP均是影响COVID-19病人预后生存状况的独立危险因素(P<0.05)。结论COVID-19病人血清降钙素原、CRP水平均较高,与病人病情严重程度及预后均密切相关,或可成为评估病人病情及预后的辅助指标。
Objective To investigate the expressions of procalcitonin and C reactive protein(CRP)in serum of patients with coronavirus pneumonia(COVID-19)and their prognostic values.Methods A total of 119 patients with COVID-19 who were admitted to Sichuan Mianyang 404 Hospital from February 2020 to March 2020 were studied,the clinical data,levels of serum procalcitonin,C-reactive protein and prognosis were retrospectively analyzed,and the patients with clinical classification of mild and common types were classified as mild group(93 cases),and the patients with severe and critical type were classified as severe group(26 cases),according to the prognosis of 30 days in hospital,the patients were assigned into survival group(112 cases)and death group(7 cases).The level of serum procalcitonin was detected by immunoradiometric method,the level of serum C-reactive protein was detected by nephelometry,and the results were compared,in addition,the prognostic factors of patients with COVID-19 were analyzed by Logistic regression model.Results There was no significant difference in sex,fever,cough,diarrhea,fatigue and headache between mild group and severe group(P>0.05).The age,the proportions of hypertension,diabetes,coronary heart disease,chronic obstructive pulmonary disease and dyspnea,the levels of serum procalcitonin[(0.52±0.11)μg/L vs.(0.13±0.03)μg/L]and C-reactive protein[(62.53±13.26)mg/L vs.(15.91±2.79)mg/L]in the severe group were higher than those in the mild group(P<0.05);the levels of procalcitonin[1.01(0.95,1.12)μg/L vs.0.17(0.14,0.20)μg/L]and C-reactive protein[96.27(89.94,96.27)mg/L vs.21.96(17.14,26.51)mg/L]in the death group were higher than those in the survival group(P<0.05);in addition,logistic regression model analysis showed that age≥60 years old,combined with basic diseases,dyspnea,high level of procalcitonin,high level of C-reactive protein were independent risk factors for the prognostic survival of patients with COVID-19(P<0.05).Conclusion The levels of serum procalcitonin and C-reactive protein are higher in patients with COVID-19,which are closely related to the severity and prognosis of patients,or can be used as auxiliary indexes to evaluate the condition and prognosis of patients.
