摘要
目的研究连续14 d滴注参麦注射液后原人参二醇型皂苷类成分(人参皂苷Rb_(1)、人参皂苷Rc和人参皂苷Rd)在稳定型心绞痛患者体内的药代动力学。方法20例稳定型心绞痛患者静脉滴注参麦注射液,连续给药14 d,每天给药1次。于给药后不同时间点采集血浆样品,采用液质联用法检测人参皂苷血药浓度,绘制C-T曲线,计算药代动力学参数。结果人参皂苷Rb_(1)、人参皂苷Rc和人参皂苷Rd药动学参数分别为t_(1/2)(93.81±20.40)h,(67.70±45.60)h,(116.9±33.73)h;C_(max)(20.11±20.12)mg·L^(-1),(10.08±10.86)mg·L^(-1),(4.07±5.03)mg·L^(-1);AUC_(0-24 h):(653.82±50.52)mg·h·L^(-1),(309.3±26.81)mg·h·L^(-1),(59.93±52.23)mg·h·L^(-1)。结论连续注射参麦注射液14 d后,原人参二醇型皂苷成分体内存在明显蓄积,提示临床医生应制定合理的用药方案,同时为后续参麦注射剂深入研究提供方向。
Objective To study the changes of pharmacokinetics of protopanaxadiol type ginsenoside(Rb_(1),Rc and Rd)in subjects with stable angina pectoris after continuous instillation of Shenmai injection.Methods 10 subjects with stable angina pectoris was administered with shenmai injection.A total of stable angina pectoris were selected and received Shenmai injection for 14 days.Plasma samples were collected at different time points.Plasma concentrations of ginsenoside were determined by liquid chromatography-mass spectrometry(LC/MS).The concentration-time curves(AUC)were drawn,and then the PK parameters were calculated.Results The pharmacokinetics parameters of Rb_(1),Rc and Rd were as follows:t_(1/2):(93.810±20.40)h,(67.70±45.60)h,(116.9±33.728)h;C_(max):(20.11±20.12)mg·L^(-1),(10.08±10.86)mg·L^(-1),(4.07±5.03)mg·L^(-1);AUC_(0-24):(653.8±50.52)mg·h·L^(-1),(309.3±26.81)mg·h·L^(-1),(59.93±52.23)mg·h·L^(-1).Conclusion After 14 days of continuous injection of Shenmai injection,the original panaxadiol saponins showed some accumulation in 10 patients with stable angina.It can provide medication reference for clinical medical staff and make effective medication plan.Meanwhile,it suggests possibilities for further drug development of Shenmai.
