摘要
目的研究探讨信息-动机-行为延续护理模式联合参芪复脉汤对心力衰竭患者院外自护能力的影响。方法选取2019年7月—2021年7月医院收治的94例心力衰竭患者为研究对象,随机数字表分为两组,各47例。对照组以参芪复脉汤+常规护理干预,观察组在对照组基础上增加信息-动机-行为延续护理模式干预。对两组患者的心功能指标、血液流变学指标、自护能力评分、护理满意度等进行统计比较。结果治疗后观察组左室射血分数(LVEF)、每搏输出量(SV)均高于对照组,左心室舒张末期内径(LVEDD)低于对照组(P<0.05)。治疗后观察组全血黏度、血浆黏度、红细胞变形指数、纤维蛋白原水平均低于对照组(P<0.05)。治疗干预后观察组自护能力评分包括症状管理、饮食管理、药物管理、心理管理评分均高于对照组(P<0.05)。观察组护理满意度为91.49%(43/47),高于对照组患者为74.47%(35/47)(P<0.05)。结论信息-动机-行为延续护理模式联合参芪复脉汤对心力衰竭患者的干预效果良好,能够改善患者的心功能和血液流变学,提高患者的院外自护能力,改善患者的护理满意度,值得临床推荐。
Objective To study the effect of information motivation behavior continuous nursing model combined with Shenqi Fumai Decoction(参芪复脉汤)on out of hospital self-care ability of patients with heart failure.Methods Ninety-four patients with heart failure treated in the hospital from July 2019 to July 2021 were randomly divided into observation group and control group,with 47 cases in each group.The control group was treated with Shenqi Fumai Decoction and routine nursing intervention,and the observation group was treated with information motivation behavior continuous nursing intervention on the basis of the control group.The indexes of cardiac function,hemorheology,self-care ability score and nursing satisfaction were statistically compared between the two groups.Results After treatment,the left ventricular ejection fraction(LVEF)and stroke output(SV)in the observation group were higher than those in the control group,and the left ventricular end diastolic diameter(LVEDD)was lower than that in the control group(P<0.05).After treatment,the whole blood viscosity,plasma viscosity,erythrocyte deformation index and fibrinogen in the observation group were lower than those in the control group(P<0.05).After treatment intervention,the scores of self-care ability including symptom management,diet management,drug management and psychological management in the observation group were higher than those in the control group(P<0.05).The nursing satisfaction of the observation group was 91.49%(43/47),which was higher than that 74.47%(35/47)of the control group(P<0.05).Conclusion The information motivation behavior continuous nursing model combined with Shenqi Fumai Decoction has a good intervention effect on patients with heart failure.It can improve the cardiac function,the hemorheology,the out of hospital self-care ability and the nursing satisfaction of patients,which is worthy of clinical recommendation.
