摘要
目的:探讨治疗盆腔脏器脱垂三种手术方式的临床应用价值。方法:纳入某医院收治的107例盆腔脏器脱垂患者,根据手术方式分为三组,经阴道全子宫切除+阴道前后壁修补者(传统组)40例、经阴道前壁盆底重建及尿道中段悬吊术(重建组)24例、腹腔镜子宫悬吊术+阴道后壁修补术(悬吊组)43例,比较各组的治疗效果。结果:三组患者的手术时间、术中出血量以及术后6~12个月的PFIQ-7评分、PFDI-20评分比较,差异均有统计学意义(P<0.05);三组患者的排气时间、尿管留置时间、住院时间比较,差异均无统计学意义(P>0.05)。结论:治疗盆腔脏器脱垂的三种手术方式均有较好的临床疗效,对于盆腔器官脱垂严重影响生活质量并有手术要求的患者,应根据患者子宫脱垂程度及有无合并膀胱直肠膨出,是否合并尿失禁、排尿或排便困难等具体情况制定个体化手术治疗方式。
Objective:This paper studies the clinical value of three surgical methods for the treatment of pelvic organ prolapse.Methods:107 patients with pelvic organ prolapse in hospital were divided into three groups according to the surgical methods:40 cases of transvaginal hysterectomy plus repair of anterior and posterior vaginal wall(traditional group),24 cases of transvaginal anterior pelvic floor reconstruction and Middle Urethral Suspension(reconstruction group),and 43 cases of laparoscopic uterine suspension plus repair of posterior vaginal wall(suspension group).The therapeutic effects of each group were compared.Results:There were significant differences in operation time,intraoperative bleeding,pfiq-7 score and pfdi-20 score 6-12 months after operation among the three groups(P<0.05);There was no significant difference in exhaust time,urinary catheter retention time and hospitalization time among the three groups(P>0.05).Conclusion:three surgical methods for the treatment of pelvic organ prolapse have good clinical efficacy.For patients with pelvic organ prolapse that seriously affects the quality of life and requires surgery,individualized surgical treatment should be formulated according to the degree of uterine prolapse,the presence or absence of cystocele,urinary incontinence,and difficulty in urinating or defecation.
