摘要
目的:了解县级中医院住院患者的感染趋势,为合理制定医院感染防控制度提供依据。方法:以回顾性调查的方式,对某县级中医院2021年度住院患者医院感染病例进行统计分析。结果:2021年度该院出院病人共14683例,发生医院感染202例,医院感染发生率为1.38%;其中ICU感染率最高(5.88%),其次为内四科(1.86%)和骨二科(1.51%)。感染部位以下呼吸道感染占比最高(47.03%),其次为切口感染(9.90%)、上呼吸道感染(9.40%)。感染病例总病原学送检率为29.21%。感染细菌以革兰阴性菌为主(53.85%),主要为大肠埃希菌、肺炎克雷伯菌;革兰阳性菌(38.46%)主要为金黄色葡萄球菌。结论:该院需加强对重点科室、重点部位、重点病原菌的监控,提高院感病例病原学送检率,对医院感染进行进一步防控。
Objective:This paper investigates the infection trend of inpatients in a county-level traditional Chinese medicine hospital,and to provide a basis for rationally formulating a nosocomial infection prevention and control system.Methods:A retrospective survey was used to analyze the nosocomial infection cases of inpatients in a county-level traditional Chinese medicine hospital in 2021.Results:In 2021,202 cases of hospital infection occurred in 14683 discharged patients in the hospital with the incidence rate of 1.38%.Among them,the ICU infection rate was the highest(5.88%),followed by the Fourth Department of Internal Medicine(1.86%)and the Second Department of Orthopaedics(1.51%).As for the infection parts,the lower respiratory tract infection accounted for the highest proportion(47.03%),followed by incision infection(9.90%)and upper respiratory tract infection(9.40%).The total etiological inspection rate of infected cases was 29.21%.Gram-negative bacteria(53.85%)were the major infecting bacteria,mainly including Escherichia coli,Klebsiella pneumoniae;Grampositive bacteria(38.46%)mainly contains Staphylococcus aureus.Conclusion:The hospital needs to strengthen the monitoring of key departments,key parts and key pathogens,improve the rate of etiological inspection of nosocomial cases,then further prevent and control nosocomial infection.
