剪切波弹性模量参数对乳腺结节性质的诊断效能分析

摘要 目的分析实时剪切波弹性成像(SWE)下的弹性模量参数对乳腺结节性质的诊断效能。方法回顾性分析江南大学附属医院2019年8月-2021年6月经病理证实的乳腺结节129例(129个结节)患者的临床资料。所有患者均行SWE检查,比较良恶性结节的图像特征及模量参数Eratio、Emax,并绘制ROC曲线分析Eratio、Emax预测乳腺良、恶性的诊断效能。结果129例(共129个病灶)经穿刺活检或手术病理结果显示良性结节88个(68.2%),最大径(13.7±4.7)mm,其中纤维腺瘤39例(占44.3%);恶性病灶41个(占31.8%),最大长径(20.9±4.8)mm,其中浸润性导管癌28例(占68.3%);乳腺良性结节患者SWE图像的典型特点为图像呈均匀蓝色,质地偏软,仅病灶周边及内部出现少许局灶的异常彩色,恶性结节图像呈红色为主的黄绿色相间覆盖或周边形成红色为主的“硬环”征,质地偏硬,弹性模量值较高;乳腺恶性结节患者的弹性模量Emax值及Eratiao值均大于良性结节患者(P<0.05);ROC曲线显示当Eratio取3.34时,诊断恶性结节的敏感性87.8%,特异性84.1%,准确率85.3%,AUC为0.872;Emax值取62.1kPa时,诊断恶性结节的敏感性78.0%,特异性81.8%,准确率80.6%,AUC为0.826。两者AUC比较,差异无统计学意义(Z=0.881,P>0.05)。结论SWE的弹性模量值和弹性图像可反映乳腺组织的硬度值既组织硬度分布特征,Eratio、Emax对乳腺恶性结节的诊断效能均较高。 Objective To analyze the diagnostic efficacy of elastic modulus parameters under real-time shear wave elastography(SWE)for the properties of breast nodules.Methods The clinical data of 129 patients(129 nodules)with pathologically confirmed breast nodules in our hospital from August 2019 to June 2021 were analyzed retrospectively.SWE examination was performed on all.The image features and the modulus parameters Eratio and Emax of benign and malignant nodules were compared,and the ROC curve was drawn to analyze the diagnostic efficacy of Eratio and Emax in predicting benign and malignant breast.Results In 129 cases(a total of 129 lesions),biopsy or surgical pathology showed 88 benign nodules(68.2%)with the largest diameter of(13.7±4.7)ran,among which 39 cases(44.3%)were fipollenoma,41 cases(31.8%)were malignant lesions,with the largest diameter of(20.9±4.8)mn.Among than,28 cases(68.3%)were infiltrating ductal carcinoma;the typical characteristics of the SWE images of benign breast nodules are that the images are uniform blue and soft in texture,with only a few focal abnormal colors around and inside the lesion.The images of malignant nodules are mainly covered with yellow-green intersections or formed a red"hard ring"sign around the lesion,with hard texture and high elastic modulus;Emax and Eratiao values of elastic modulus in patients with malignant breast nodules were higher than those of benign nodules(P<0.05);R0C curve showed that when Eratio was 3.34,the sensitivity,specificity,accuracy,and AUC were 87.8%,84.1%,85.3%,0.872,and Emax value was 62.lkPa,the sensitivity,specificity,and accuracy were 78.0%,81.8%,and 80.6%,respectively.AUC was 0.826;there was no significant difference in AUC between the two groups(Z=0.881,P>0.05).Conclusion The elastic modulus value and elastic image of SWE can reflect the hardness value of breast tissue,as well as the distributicn characteristics of tissue hardness.Both Eratio and Emax have high diagnostic efficiency for malignant breast nodules.
