还原型谷胱甘肽联合低分子肝素治疗慢性肺源性心脏病急性加重期的效果 被引量:2

Effect of reduced glutathione combined with low molecular weight heparin in the treatment of acute exacerbation of chronic cor pulmonale

摘要 目的探讨还原型谷胱甘肽联合低分子肝素治疗慢性肺源性心脏病(CCP)急性加重期的临床效果。方法选择2020年11月至2021年11月阳江江华医院心血管内科的CCP急性加重期患者90例,采用随机数字表法分为对照组与联合组,每组各45例,对照组接受低分子肝素治疗,联合组在对照组基础上接受还原型谷胱甘肽治疗。疗程结束后评价两组疗效,治疗前后氧化/抗氧化指标[脂质过氧化物(LPO)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)]、血流动力学指标[右心室压(RVP)与平均肺动脉压(mPAP)]及安全性。结果联合组治疗总有效率高于对照组,差异有统计学意义(P<0.05);治疗前两组LPO、SOD、GSH-Px水平比较,差异无统计学意义(P>0.05),治疗后联合组LPO低于对照组,SOD、GSH-Px高于对照组,差异有统计学意义(P<0.05);治疗前两组RVP、mPAP水平比较,差异无统计学意义(P>0.05),治疗后联合组RVP、mPAP水平低于对照组,差异有统计学意义(P<0.05);两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论还原型谷胱甘肽联合低分子肝素治疗CCP急性加重期疗效优于单独使用低分子肝素,能有效改善患者机体抗氧化能力与血流动力学,值得临床应用与推广。 Objective To investigated the clinical effect of reduced glutathione combine with low molecular weight heparin in the treated of acute exacerbation of chronic cor pulmonale(CCP).Methods A total of 90 patients with acute exacerbation of CCP in the Department of Cardiovascular Medicine of Yangjiang Jianghua Hospital from November 2020 to November 2021 were selected,divided into the control group and the combined group by random number table method,45 cases in each group,the control group received low molecular weight heparin therapy,and the combined group received reduced glutathione therapy on the basis of the control group.After treated,the curative effects of the two groups were evaluated,and the oxidation/antioxidant indexes(lipid peroxide[LPO],superoxide dismutase[SOD],glutathione peroxidase[GSH-Px]),hemodynamics(right ventricular pressure[RVP]and mean pulmonary arterial pressure[mPAP])and safety.Results The total effective rate of the combined group was higher than control group and the difference was significant(P<0.05).The levels of LPO,SOD and GSH-Px were not significantly different between the two groups before treated(P>0.05),and the LPO of the combined group was lower than control group after treated,SOD and GSH-Px were higher than control group and the difference was significant(P<0.05).Before treated,there was no significant difference in the levels of RVP and mPAP between the two groups(P>0.05).After treated,the levels of RVP and mPAP in the combined group were lower than control group and the difference was significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Reduced glutathione combine with low molecular weight heparin is more effective than low molecular weight heparin alone in treating acute exacerbation of CCP,and can effectively improving the antioxidant capacity and hemodynamics of patients,which is worthy of clinical applicati on and promotion.
