肾移植患者血清miR-338-5p水平表达及其临床意义研究

摘要 目的:观察血清miR-338-5p水平在肾移植患者中的表达情况,并分析血清miR-338-5p水平在肾移植患者中表达的临床意义。方法:选取2017年7月—2019年8月河南中医药大学第一附属医院收治的100例接受肾移植手术患者作为研究对象,依据肾移植术后是否出现排斥反应进行分组,将出现排斥反应的36例患者的临床资料归为观察组,将未出现排斥反应的64例患者的临床资料归为对照组。测定并比较两组研究对象的血清miR-338-5p水平、可溶性B细胞活化因子(sBAFF)水平,调查并记录肾移植患者的临床资料,并重点分析血清miR-338-5p水平与sBAFF水平的相关性。结果:观察组血清miR-338-5p水平低于对照组,sBAFF水平高于对照组,差异有统计学意义(t=13.337、43.472,P<0.05)。经Pearson分析,血清miR-338-5p与sBAFF呈负相关,差异有统计学意义(r=-0.677,P=0.000)。结论:肾移植患者血清miR-338-5p可通过调节BAFF信号来参与肾移植术后免疫介导的反应过程,临床上可通过检测miR-338-5p水平,来预测肾移植患者术后排斥反应发生的风险性。 Objective: To observe the expression of serum miR-338-5 level in patients with renal transplantation, and to analyze the clinical significance of expression of serum miR-338-5p level in patients with renal transplantation. Methods: From July 2017 to August 2019, 100 patients who received kidney transplantation in the hospital were selected as the research subjects, and were divided into groups according to whether there was rejection after kidney transplantation. The clinical data of 36 patients with rejection were classified as observation group, and the clinical data of 64 patients without rejection were classified as control group.The serum levels of miR-338-5p and soluble B cell activating factor(sBAFF) were measured and compared between the two groups of subjects. The clinical data of kidney transplant patients were investigated and recorded, and the correlation between serum miR-338-5p level and s BAFF level was analyzed. Results: The serum level of miR-338-5p in the observation group was lower than that in the control group, and the level of sBAFF was higher than that in the control group, and the differences were statistically significant(t=13.337, 43.472, P<0.05). According to Pearson analysis, serum miR-338-5p was negatively correlated with sBAFF, and the difference was statistically significant(r=-0.677, P=0.000). Conclusion: Serum miR-338-5p may participate in immune-mediated response after renal transplantation by regulating sBAFF signal, in clinical practice, the level of miR-338-5p can be used to predict the risk of rejection reaction in patients after renal transplantation.
