摘要
目的观察尖锐湿疣患者外周血干扰素-γ(IFN-γ)、白介素-2(IL-2)、白介素-10(IL-10)、CD4+/CD_(8)^(+)水平在疗程中的变化及其与疾病复发的关系。方法选取天津市中医药研究院附属医院2020年1月至2021年1月收治的100例尖锐湿疣患者作为研究对象,均采取5-氨基酮戊酸光动力联合重组人干扰素α-2b凝胶治疗3周,治疗后随访6个月,依据复发情况分为复发组(n=26)、未复发组(n=74)。比较两组基线资料、治疗前及治疗3周后外周血IFN-γ、IL-2、IL-10、CD4+/CD_(8)^(+)水平,分析各指标对尖锐湿疣疾病复发的预测价值。结果两组治疗3周后外周血IFN-γ、IL-2、CD4+/CD_(8)^(+)水平均较治疗前增高,IL-10水平均较治疗前降低(P<0.05);复发组治疗前、治疗3周后外周血IFN-γ、IL-2、CD4+/CD_(8)^(+)水平均低于未复发组,IL-10水平高于未复发组(P<0.05)。绘制受试者工作特征(ROC)曲线显示,治疗3周后外周血IFN-γ、IL-2、IL-10、CD4+/CD_(8)^(+)预测尖锐湿疣复发均有较大的曲线下面积(AUC),各指标联合预测时AUC最大,联合预测时的最佳灵敏度为84.62%,特异度为87.84%。结论尖锐湿疣患者外周血IFN-γ、IL-2、IL-10、CD4+/CD_(8)^(+)水平在疗程中可产生明显变化,动态监测上述指标水平可为疾病复发的预测提供有力参考。
Objective To observe the changes of the levels of interferon-γ(IFN-γ),interleukin-2(IL-2),interleukin-10(IL-10),and CD_(4)^(+)/CD_(8)^(+)in peripheral blood of condyloma acuminatum during the course of treatment and its relationship with the recurrence of the disease.Methods 100 patients with condyloma acuminatum admitted to Affiliated Hospital of Tianjin Academy of Traditional Chinese Medicine from January 2020 to January 2021 were selected as the research objects,and were treated with 5-aminolevulinic acid photodynamic combined with recombinant human interferonα-2b gel for 3 weeks,and were followed up for 6 months.According to the recurrence,they were divided into recurrence group(n=26)and non-recurrence group(n=74).The general data,and the levels of peripheral blood IFN-γ,IL-2,IL-10,CD_(4)^(+)/CD_(8)^(+)before and 3 weeks after treatment were compared between the two groups.The predictive value of each index for the recurrence of condyloma acuminatum was analyzed.Results Peripheral blood IFN-γ,IL-2,CD_(4)^(+)/CD_(8)^(+)levels at 3 weeks after treatment were higher than before treatment in the two groups,and IL-10 levels were lower than before treatment(P<0.05).The levels of IFN-γ,IL-2,CD_(4)^(+)/CD_(8)^(+)in peripheral blood of the recurrence group before treatment and 3 weeks after treatment were lower than those of the non-recurrence group,and the level of IL-10 was higher than that of the non-recurrence group(P<0.05).The receiver operating characteristic(ROC)curve showed that IFN-γ,IL-2,IL-10 and CD_(4)^(+)/CD_(8)^(+)in peripheral blood had a larger area under the curve(AUC)in predicting the recurrence of condyloma acuminatum after 3 weeks of treatment.The AUC of the combined prediction with all indicators was the largest,and the best sensitivity and specificity were 84.62%and 87.84%,respectively.Conclusions Peripheral blood IFN-γ,IL-2,IL-10,CD_(4)^(+)/CD_(8)^(+)levels in condyloma acuminatum can change significantly,and dynamic monitoring of the above index levels can provide strong support for the prediction on disease recurrence.
