嵊州地区女性人乳头瘤病毒感染状况和宫颈病变的关系分析 被引量:3

Analysis of the relationship between human papilloma virus infection and cervical lesions in women in Shengzhou area

摘要 目的了解嵊州地区女性人乳头瘤病毒(HPV)的感染状况,并探讨感染不同亚型HPV与宫颈病变程度的关系。方法收集2020年6月至2021年6月在嵊州市中医医院接受宫颈HPV检查的3000例女性作为研究对象,分析其HPV感染率、基因亚型分布及年龄分布特征。根据宫颈病理结果将有HPV感染的女性分为宫颈炎组(n=161)、宫颈上皮内瘤变Ⅰ级组(CINⅠ组,n=102)、宫颈上皮内瘤变Ⅱ级组(CINⅡ组,n=109)、宫颈上皮内瘤变Ⅲ级组(CINⅢ组,n=96)、宫颈原位癌组(n=15),分析HPV各亚型感染与宫颈病变的关系。结果3000例女性中23种HPV亚型均有检出,HPV感染率为16.10%。483例阳性标本中,感染率前5位的HPV亚型是HPV 16(13.62%)、52(11.28%)、58(9.47%)、18(9.08%)、33(8.43%)。25~29岁年龄段高危型HPV检出率最高,40~49岁年龄段高危型HPV检出率最低。宫颈炎组高危型HPV检出率、多重感染率最低,宫颈原位癌组高危型HPV检出率、多重感染率最高(P<0.05)。结论嵊州地区女性HPV感染常见的基因型为HPV 16、52、58、18、33,高危型HPV感染与宫颈癌的发生有关。 Objective To understand the status of human papilloma virus(HPV)infection in Shengzhou area,and to explore the relationship between different subtypes of HPV infection and the degree of cervical lesions.Methods 3,000 cases of HPV infection women who were examined in Shengzhou Hospital of Traditional Chinese Medicine from June 2020 to June 2021 as the research objects,and the HPV infection rate,genotype distribution and age distribution characteristics were analyzed.According to pathological biopsy,the subjects were divided into cervicitis group(n=161),low-grade cervical intraepithelial neoplasia group(CINⅠgroup,n=102),and moderate-grade cervical intraepithelial neoplasia group(CINⅡgroup,n=109),high-grade cervical intraepithelial neoplasia group(CINⅢgroup,n=96),cervical cancer in situ group(n=15).The relationship between HPV subtypes infection and cervical lesions was analyzed.Results 23 subtypes were detected in 3,000 women,the HPV positive rate was 16.10%.Among the 483 positive samples,the top 5 HPV subtypes in the infection rate were HPV 16(13.62%),52(11.28%),58(9.47%),18(9.08%),33(8.43%).The infection rate of high-risk HPV was highest in the 25-29 years old age group,which however was lowest in the 40-49 years old age group.The detection rates of high-risk HPV and multi-subtype HPV were the lowest in the cervicitis group,which were the highest in the cervical cancer in situ group(P<0.05).Conclusions The common genotypes of HPV infection in Shengzhou area are HPV 16,52,58,18,33,and there is a positive correlation between high-risk HPV and cervical cancer.
