摘要
目的:探究基于鱼骨图的风险管理在乳腺癌根治术(Radical Mastectomy,RM)后完全植入式静脉输液港(totally implantable venous access port,TIVAP)患者中的应用价值。方法:将2019年2月至2020年1月期间我科98例行RM手术并建立TIVAP的患者为研究对象,其中48例给予常规管理,作为对照组;50例予以风险管理,作为观察组。对比两组患者护理质量,并发症发生情况。结果:干预后,观察组护理质量量表总分及4个维度评分均高于对照组(P<0.05)。观察组患者静脉输液港相关知识知晓率明显高于对照组(P<0.05)。观察组堵管、感染、局部皮疹等TIVAP并发症发生率均低于对照组(P<0.05)。结论:基于鱼骨图的风险管理应用于URM术后建立TIVAP的患者中,能够提高护理质量,降低堵管及感染发生率,值得临床推广和应用。
Objective:to explore the effect of fishbone diagram based risk management on nursing quality and infection prevention in patients with totally implantable venous access port(TIVAP)after radical mastectomy.Methods:98 patients with TIVAP after radical mastectomy from August 2019 to January 2020 were selected as the research objects,48 patients in control group were treated with routine management,and 50 patients in observation group were treated with risk management.The nursing quality,complication rate were compared.Results:After intervention,total score of nursing quality and four dimension scores in observation group were higher than control group(P<0.05).Compared with control group,the awareness of wound care,needle insertion,flushing knowledge,pain care,rest activities,cooperative operation,abnormal identification in observation group was obviously higher(P<0.05).The rate of TIVAP complications including tube blockage,infection and local rash in observation group was significantly lower than that in control group(P<0.05).Conclusion:fishbone diagram based risk management can improve the nursing quality and reduce incidence of tube blockage and infection in patients with TIVAP after radical mastectomy.
