摘要
目的 观察乌司他丁联合生长抑素治疗急诊重症急性胰腺炎的临床疗效。方法 选取2019年7—12月于本院接受治疗的42例急性胰腺炎患者,采用随机数字表法分为对照组和治疗组,每组21例。对照组单纯采用生长抑素治疗,治疗组采用乌司他丁联合生长抑素治疗,比较两组上腹压痛减轻时间、腹痛消失时间及血淀粉酶恢复正常时间和治疗前后炎症因子[白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、白细胞介素-10(IL-10)、肿瘤坏死因子(TNF-α)]水平及临床疗效。结果 治疗组上腹压痛减轻时间、腹痛消失时间及血淀粉酶恢复正常时间均短于对照组(P<0.05)。治疗后,治疗组IL-6、IL-8、TNF-α水平均低于对照组,IL-10水平高于对照组(P<0.05)。治疗组治疗总有效率为90.48%,高于参照组的61.90%(P<0.05)。结论 乌司他丁联合生长抑素治疗重症急性胰腺炎能高效缓解症状,减轻炎症反应,有助于患者快速恢复健康,值得临床推广应用。
Objective To observe the clinical effect of ulinastatin combined with somatostatin in the treatment of emergency severe acute pancreatitis. Methods A total of 42 patients with acute pancreatitis treated in our hospital from July to December 2019 were selected and randomly divided into control group and treatment group according to random number table method, with 21 cases in each group. The control group was treated with somatostatin alone, and the treatment group was treated with ulinastatin combined with somatostatin, the time to epigastric tenderness, time to disappearance of abdominal pain and time for blood amylase to return to normal, and inflammatory factors(interleukin-6 [IL-6], interleukin-8 [IL-8], interleukin-10 [IL-10] and tumor necrosis factor [TNF-α]) levels before and after treatment and clinical efficacy were compared between the two groups.Results The time to relieve epigastric tenderness, disappearance of abdominal pain and time for blood amylase to return to normal in the treatment group were shorter than those in the control group(P<0.05). After treatment, the levels of IL-6, IL-8 and TNF-α in the treatment group were lower than those in the control group, the level of IL-10 was higher than that in the control group(P<0.05). The total effective rate in the treatment group was90.48%, which was higher than 61.90% in the control group(P<0.05). Conclusion Ulinastatin combined with somatostatin in the treatment of severe pancreatitis can effectively relieve symptoms, reduce the inflammation, and help patients recover quickly, which is worthy of clinical application.
