摘要 目的探讨神经电生理检测对足下垂定位的鉴别诊断价值。方法15例足下垂患者,均进行神经电生理检测。统计患者病因,分析患者神经传导速度(NCV)及肌电图(EMG)检测异常率。结果依据神经电生理检查诊断为腓总神经损害6例,L5神经根损害4例,L5及S1神经根损害3例,多发性周围神经损害1例,肌源性损害1例。15例患者NCV检测异常率为100.0%(15/15),其中腓总神经13例运动神经传导速度(MNCV)明显减慢,复合肌肉动作电位(CMAP)波幅低,未引出3例;胫后神经MNCV明显减慢1例(同时伴腓总神经CMAP未引出)。7例腓浅神经感觉神经传导速度(SNCV)减慢或感觉神经动作电位(SNAP)波幅低或未引出;1例腓肠神经SNAP波幅低伴SNCV减慢。15例患者中,EMG检测异常率为100.0%(15/15)。结论神经电生理检测能直接反映神经功能受损状况,并进行定位诊断,对病因有提示作用,是临床诊断延伸的重要手段。 Objective To investigate the differential diagnostic value of neurophysiological testing for the localization of foot drop.Methods 55 patients with foot drop were subjected to neurophysiological testing.The etiology of the patients was counted,and the abnormal rate of nerve conduction velocity(NCV)and electromyography(EMG)was analyzed.Results According to neurophysiological testing,6 cases of common peroneal nerve damage,4 cases of L5 nerve root damage,3 cases of L5 and S1 nerve root damage,1 case of multiple peripheral nerve damage,and 1 case of myogenic damage were diagnosed.The abnormal rate of NCV in 15 patients was 100.0%(15/15),of which 13 patients with common peroneal nerve had significantly slow motor nerve conduction velocity(MNCV)and low compound muscle action potential(CMAP)amplitude,and 3 patients were not elicited;posterior tibial nerve MNCV was significantly slowed down in 1 case(with the common peroneal nerve CMAP not elicited at the same time).The sensory nerve conduction velocity(SNCV)of the superficial peroneal nerve was slowed or the sensory nerve action potential(SNAP)wave amplitude was low or not elicited in 7 cases;the SNAP wave amplitude of the peroneal nerve was low with slowed SNCV in 1 case.The abnormal EMG rate was 100.0%(15/15)in 15 patients.Conclusion Neurophysiological testing can directly reflect the state of impaired nerve function and make localized diagnosis,which is suggestive of the etiology and is an important tool for clinical diagnosis extension.
