摘要
目的分析心脏彩色多普勒超声在高血压心脏病诊断工作中的应用价值。方法选取132例高血压心脏病患者作为研究组,另选取同期在医院接受体检的130例健康志愿者作为对照组。两组受检者均实施心电图检查和心脏彩色多普勒超声检查。比较不同方法对心脏异常的检出率以及两组受检者经心脏彩色多普勒超声诊断心脏结构功能相关指标[左房内径(LAD)、左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)、左室射血分数(LVEF)、左室心肌质量(LVMM)、左室后壁厚度(LVPWT)、室间隔厚度(IVST)、二尖瓣口血流舒张早期最大流速与心房收缩期最大流速之比(E/A)、主动脉瓣流速(AV)]。结果心脏彩色多普勒超声对高血压心脏病患者的左房扩大、左室肥厚检出率分别为96.21%、92.42%,显著高于心电图检查的32.58%、66.67%,差异有统计学意义(P<0.05)。研究组的LAD(45.09±5.31)mm、LVESD(31.97±1.49)mm、LVEDD(51.09±5.65)mm、LVMM(165.31±35.12)g、LVPWT(12.72±1.38)mm、IVST(12.87±1.28)mm均显著大于对照组的(34.09±2.87)mm、(26.85±1.20)mm、(43.09±4.14)mm、(124.24±20.01)g、(8.27±1.29)mm、(9.14±1.31)mm,LVEF(55.13±12.84)%、E/A(0.72±0.17)、AV(118.98±21.87)cm/s显著低于对照组的(65.02±20.52)%、(1.15±0.25)、(126.90±24.09)cm/s,差异均有统计学意义(P<0.05)。结论心脏彩色多普勒超声是诊断高血压心脏病的有效方法。
Objective To analyze the application value of color Doppler echocardiography in the diagnosis of hypertensive heart disease.Methods 132 patients with hypertensive heart disease were selected as subjects of the study group,and 130 healthy subjects who received physical examination in the hospital in the same period were selected as subjects of the control group.Subjects of both groups received electrocardiographic examination and color Doppler echocardiography.The detection rate of cardiac abnormalities with different methods and the indexes related to cardiac structure and function[left atrial diameter(LAD),left ventricular end systolic diameter(LVESD),left ventricular end diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),left ventricular myocardial mass(LVMM),left ventricular posterior wall thickness(LVPWT),and interventricular septal thickness(IVST),the ratio of the early diastolic maximum velocity of mitral orifice flow to the atrial systolic maximum velocity(E/A),aortic valve velocity(AV)]were compared between the two groups.Results The detection rates of left atrial enlargement and left ventricular hypertrophy in patients with hypertensive heart disease with color Doppler echocardiography were 96.21%and 92.42%,respectively,which were significantly higher than 32.58%and 66.67%with electrocardiographic examination,and the differences were statistically significant(P<0.05).The study group had LAD of(45.09±5.31)mm,LVESD of(31.97±1.49)mm,LVEDD of(51.09±5.65)mm,LVMM of(165.31±35.12)g,LVPWT of(12.72±1.38)mm,IVST of(12.87±1.28)mm,which were significantly higher than(34.09±2.87)mm,(26.85±1.20)mm,(43.09±4.14)mm,(124.24±20.01)g,(8.27±1.29)mm,(9.14±1.31)mm the control group,respectively;the study group had LVEF of(55.13±12.84)%,E/A of(0.72±0.17),AV of(118.98±21.87)cm/s,which were significantly lower than(65.02±20.52)%,(1.15±0.25),(126.90±24.09)cm/s of the control group,respectively;and the differences were statistically significant(P<0.05).Conclusion Color Doppler echocardiography is an effective method for the diagnosis of hypertensive heart disease.
