支持性心理干预联合健康宣教对慢性阻塞性肺疾病患者心境状态和疾病认知的影响 被引量:18

Effects of supportive psychological intervention combined with health education on mood state and disease cognition of patients with chronic obstructive pulmonary disease

摘要 目的:探究支持性心理干预联合健康宣教对慢性阻塞性肺疾病(COPD)患者心境状态和疾病认知的影响。方法:选取2020年1-12月某院收治的120例COPD患者为研究对象,随机分为对照组(n=60)和观察组(n=60),对照组给予常规干预和健康宣讲,观察组在对照组基础上给予支持性心理干预。比较两组心境状态[心境状态量表(POMS)]、疾病认知(COPD患者疾病认知调查表)、应对方式[中文版医学应对问卷(MCMQ)]、肺功能[第一秒呼气容积(FEV1)、第一秒呼气容积占用力肺活量比率(FEV1/FVC)、肺活量(VC)]及生活质量[圣乔治呼吸问卷(SRGQ)]。结果:干预后,观察组负性情绪评分明显低于对照组(t=-8.950,P<0.001),积极情绪评分明显高于对照组(t=7.333,P<0.001);观察组疾病认知评分明显高于对照组(t=10.874,4.041,5.437,16.123;P<0.001);观察组面对应对评分明显高于对照组(t=6.460,P<0.001),回避和屈服应对评分明显低于对照组(t=-8.594,-6.817;P<0.001);观察组FEV1、FEV1/FVC及VC水平均明显高于对照组(t=4.151,8.068,3.636;P<0.001);观察组SRGQ量表各部分评分明显低于对照组(t=-7.492,-8.777,-9.308;P<0.001)。结论:针对COPD患者而言,支持性心理干预联合健康宣教可明显缓解患者的心境状态,提高疾病认知能力,增强积极应对方式,改善肺功能指标水平,提升患者生活质量。 Objective:To explore the effects of supportive psychological intervention combined with health education on mood state and disease cognition in patients with chronic obstructive pulmonary disease(COPD).Methods:A total of 120 cases of COPD patients admitted to our hospital from January 2020 to December 2020 were selected as research objects,and were randomly divided into control group(n=60)and observation group(n=60).The control group was given routine intervention and health education,and the observation group was given supportive psychological intervention on the basis of the control group.Mood state[MOOD State Scale(POMS)],disease cognition(COPD patients’disease cognition Questionnaire),coping style[Chinese version of medical Coping Questionnaire(MCMQ)],lung function[first second expiratory volume(FEV1),ratio of first second expiratory volume to forced vital capacity(FEV1/FVC),vital capacity(VC)]and life quality[St.George’s Respiratory Questionnaire(SRGQ)]were compared between the two groups.Results:After intervention,negative emotion score of observation group was significantly lower thanthat of control group(t=-8.950,P<0.001),positive emotion score was significantly higher than that ofcontrol group(t=7.333,P<0.001).The cognitive score of disease in observation group were significantly higher than those in control group(t=10.874,4.041,5.437,16.123;P<0.001).The coping score of the observation group was significantly higher than that of the control group(t=6.460,P<0.001),and the coping score of avoidance and submission were significantly lower than those of the control group(t=-8.594,-6.817;P<0.001).The levels of FEV1,FEV1/FVC and VC in the observation group were significantly higher than those in the control group(t=4.151,8.068,3.636;P<0.001).The scores of all parts of SRGQ scale in the observation group were significantly lower than those in the control group(t=-7.492,-8.777,-9.308;P<0.001).Conclusion:For patients with COPD,supportive psychological intervention combined with health education can significantly relieve the patients’mood state,improve their disease cognitive ability,enhance the positive coping style,improve the level of lung function indicators,improve the quality of life of patients.
