摘要
目的探讨腰椎手术后骶髂关节相关疼痛(Sacroiliac joint pain,SIJP)与脊柱骨盆参数的关系。方法选择2017年1月~2019年10月在本院接受腰椎手术的103例患者作为研究对象,根据是否发生SIJP将患者分为SIJP组(n=27)和非SIJP组(n=76例)。观察两组患者一般资料及手术前后脊柱骨盆参数的特征。结果术前SIJP组腰椎前凸(Lumbar lordosis,LL)、骨盆倾斜角(pelvic tilt,PT)、骶骨倾斜角(sacral slope,SS)、PI-LL与非SIJP组相比,差异均无统计学意义(P>0.05),SIJP组术前骨盆投射角(pelvic incidence,PI)高于非SIJP组(P<0.05)。术后SIJP组LL、PT、SS、PI-LL与非SIJP组相比,差异均无统计学意义(P>0.05),SIJP组术后PI高于非SIJP组(P<0.05)。术前PI和术后PI对SIJP均有中等预测价值,二者之间的预测价值相比,差异无统计学意义(Z=0.635,P=0.525)。结论术前和术后较高的PI值与术后SIJP显著相关,对术前PI>56°的患者,应高度怀有SIJP发生风险。
Objective To investigate the relationship between sacroiliac joint pain(SIJP)and spine pelvic parameters after lumbar surgery.Methods From January 2017 to October 2019,103 patients who underwent lumbar surgery in our hospital were selected as the research objects.According to whether SIJP occurred,the patients were divided into SIJP group(n=27)and non-SIJP group(n=76).The general information and the characteristics of spine and pelvis parameters before and after operation were observed.Results Before operation,there were no significant differences in LL,PT,SS and PI-LL between SIJP group and non-SIJP group(P>0.05),and PI in SIJP group was higher than that in non-SIJP group(P<0.05).There were no significant differences in LL,PT,SS,PI-LL between the SIJP group and the non-SIJP group after surgery(P>0.05),and the postoperative PI in the SIJP group was higher than that in the non-SIJP group(P<0.05).Both preoperative PI and postoperative PI had moderate predictive value for SIJP,and there was no significant difference in the predictive value between the two methods(Z=0.635,P=0.525).Conclusion Higher PI values before and after operation are significantly correlated with postoperative SIJP,and patients with preoperative PI>56°have a high risk of SIJP.
