摘要
目的探究颅脑超声检查对新生儿缺氧缺血性脑病(HIE)诊断的价值。方法选择2020年2月至2021年4月儋州市人民医院收治的新生儿HIE患儿60例为研究对象,均实施颅脑CT和颅脑超声检查,比较两种检查的HIE检出率、病情严重程度的诊断符合率及基底节区出血检出率。结果颅脑CT共检出51例HIE,检出率85.00%(51/60);颅脑超声共检出58例HIE,检出率96.67%(58/60)。颅脑超声HIE检出率高于颅脑CT(χ^(2)=4.904,P<0.05)。颅脑超声病情严重程度的诊断符合率88.33%(53/60)明显高出颅脑CT符合率70.00%(42/60)(P<0.05)。颅脑超声在基底节区出血方面的检出率(100.00%)高于颅脑CT(50.00%);但在蛛网膜下腔出血方面的检出率(90.91%)低于颅脑CT(100.00%)(P<0.05)。结论与颅脑CT检查相比,颅脑超声在新生儿HIE上的检出率更高,更易检出基底节区出血情况。必要时可二者联用,进一步提升诊断价值,为后续治疗方案的制定提供一定指导。
Objective To explore the value of craniocerebral ultrasonography in the early diagnosis of neonatal hypoxic ischemic encephalopathy(HIE).Methods A total of 60 cases of neonatal HIE admitted to People′s Hospital of Danzhou City from February 2020 to April were selected as the study subjects.All of them underwent craniocerebral computed tomography(CT)and craniocerebral ultrasonography.The detection rate,the diagnostic coincidence rate of the severity of the disease and the detection rate of basal ganglia hemorrhage were compared between the two examinations.Results Fifty-one cases were detected by craniocerebral CT,with a detection rate of 85.00%(51/60),while 58 cases were detected by craniocerebral ultrasonography,with a detection rate of 96.67%(58/60).The detection rate of neonatal HIE by craniocerebral ultrasonography was higher than that by craniocerebral CT(χ^(2)=4.904,P<0.05).The coincidence rate of the severity of the disease detected by craniocerebral ultrasonography was 88.33%(53/60),which was significantly higher than that by craniocerebral CT[70.00%(42/60)](P<0.05).The detection rate of basal ganglia hemorrhage by craniocerebral ultrasonography(100.00%)was higher than that by craniocerebral CT(50.00%),but the detection rate of subarachnoid hemorrhage by craniocerebral ultrasonography(90.91%)was lower than that by craniocerebral CT(100.00%)(P<0.05).Conclusion Compared with craniocerebral CT,craniocerebral ultrasonography has a higher detection rate of neonatal HIE and basal ganglia hemorrhage.When necessary,the two can be combined to further improve the diagnostic value and provide guidance for the formulation of follow-up treatment plan.
