摘要
目的观察综合护理干预对同侧游离腕横纹穿支复合组织瓣修复手指皮肤软组织复合缺损的手术患者的临床应用效果。方法选择2018年4月至2021年6月广东医科大学附属东莞第一医院收治的同侧游离腕横纹穿支复合组织瓣修复手指皮肤软组织复合缺损的手术患者62例,随机数字表法分为对照组和观察组,各31例。对照组男24例,女7例,年龄(32.98±6.02)岁,给予常规护理方法;观察组男25例,女6例,年龄(33.97±5.83)岁,在对照组基础上给予综合护理干预。比较两组皮瓣成活及手部功能恢复情况及护理满意度。采用t检验和χ^(2)检验。结果观察组术后干预1、2周视觉模拟评分法(Visual Analogue Scale,VAS)疼痛评分均明显低于对照组(t=5.5492、8.6781,均P<0.05);观察组治愈率80.65%(25/31),对照组64.52%(20/31),差异无统计学意义(χ^(2)=2.0261,P=0.0857);观察组护理满意度96.77%(30/31),对照组74.19%(23/31)(χ^(2)=6.3690,P<0.01)。结论综合护理干预能够显著降低同侧游离腕横纹穿支复合组织瓣修复手指皮肤软组织复合缺损的手术患者VAS疼痛评分,有效预防血管危象的发生,促进皮瓣迅速愈合,提高护理满意度,值得临床推广应用。
Objective To observe the clinical effect of comprehensive nursing intervention patients with composite soft tissue defect of fingers repaired by free wrist striated perforator composite tissue flap.Methods From April 2018 to June 2021,62 patients with composite soft tissue defect of fingers repaired by free wrist striated perforator composite tissue flap at Dongguan First Hospital Affiliated to Guangdong Medical University were selected,and were divided into two a control group and an observation group by the random number table method,with 31 cases in each group.The control group had 24 males and 7 females who were(32.98±6.02)years old,and were routinely cared.The observation group had 25 males and 6 females who were(33.97±5.83)years old,and were routinely and comprehensively cared.The flap survival,hand function recovery,and nursing satisfaction were compared between the two groups.t and χ^(2) tests were applied.Results The pain scores of Visual Analogue Scale(VAS)1 and 2 weeks after the intervention in the observation group were significantly lower than those in the control group(t=5.5492 and 8.6781,both P<0.05).The cured rate was 80.65%(25/31)in the observation group,and was 64.52%(20/31)in the control group,with no statistical difference(χ^(2)=2.0261,P=0.0857).The nursing satisfaction was 96.77%(30/31)in the observation group,and was 74.19%(23/31)in the control group(χ^(2)=6.3690,P<0.01).Conclusion Comprehensive nursing intervention for patients with composite soft tissue defect of fingers repaired by free wrist striated perforator composite tissue flap can significantly reduce their VAS score,effectively prevent the occurrence of vascular crisis,promote the healing of skin flap,and improve the nursing satisfaction,so it is worth being clinical generalized.
