摘要
目的探究早期稽留流产患者(missed early miscarriage,MEM)蜕膜组织中,自然杀伤(NK)细胞表面受体NKG2A、NKG2D及其配体MICA的表达及意义。方法选取2019年6月—2021年6月广州市妇女儿童医疗中心50例在妊娠12周内诊断为稽留流产孕妇为MEM组,另外选取50例同孕周人工流产孕妇作为对照组。流产时留取蜕膜组织,检测两组蜕膜组织中NKG2A、NKG2D mRNA及蛋白的表达;另外采用免疫组化法检测蜕膜及绒毛组织中MICA表达部位。结果MEM组蜕膜组织中NKG2A mRNA及其蛋白表达水平均低于对照组,而NKG2D、MICA mRNA及其蛋白表达水平均高于对照组,差异有统计学意义(P<0.05)。NKG2A与NKG2D和MICA在mRNA及蛋白表达水平上存在负相关。MICA广泛存在于滋养细胞和蜕膜基质中。结论MEM患者蜕膜组织中,NK细胞表面受体NKG2A表达下降而NKG2D及其配体MICA表达上升,这可能导致MEM母胎界面免疫失衡;表达上调的NKG2D可能通过特异性信号蛋白MICA进行信号传递,介导免疫性MEM的发生。
Objective To explore the expression and significance of natural killer(NK)cell surface receptors NKG2A,NKG2D and their ligand MICA in the decidual tissue of patients with early missed miscarriage(MEM).Methods From June 2019 to June 2021,50 pregnant women diagnosed with delayed abortion within 12 weeks of pregnancy in Guangzhou Women and Children's Medical Center were selected as MEM group,and another 50 pregnant women with induced abortion at the same gestational age were selected as control group.The expression of NKG2A and NKG2D mRNA and protein in decidua tissues of the two groups were detected.In addition,the expression sites of MICA in decidua and villi were detected by immunohistochemistry.Results The expression levels of NKG2A mRNA and protein in decidua tissue in MEM group were lower than those in control group,while the expression levels of NKG2D and MICA mRNA and protein were higher than those in control group,the difference was statistically significant(P<0.05).The mRNA and protein expression levels of NKG2A were negatively correlated with NKG2D and MICA.MICA is widely present in trophoblast cells and decidua matrix.Conclusion In decidua tissues of MEM patients,the expression of NK cell surface receptor NKG2A decreased while the expression of NKG2D and its ligand MICA increased,which may lead to immunological imbalance at the maternal-fetal interface of MEM.The up-regulated NKG2D may mediate the occurrence of immunological MEM through specific signaling protein MICA.
