摘要
目的 探讨液基薄层细胞检测(TCT)联合高危型人乳头状瘤病毒(HR-HPV)检测在宫颈癌及癌前病变早期筛查中的应用价值。方法 纳入2019年1月至2020年12月于我院行宫颈癌及癌前病变筛查的女性181例,均行TCT及HR-HPV检查。以病理结果作为诊断“金标准”,分析TCT、 HR-HPV单项以及联合检测在疾病筛查中的价值,及其与病理结果的一致性。结果 TCT、 HR-HPV联合检测在宫颈癌及癌前病变筛查中的灵敏度、特异度、准确度及阴性预测值均高于TCT、 HR-HPV单项检测(P<0.05)。Kappa检验显示,TCT、 HR-HPV与病理检查的一致性尚可(Kappa值=0.527, P=0.000;Kappa值=0.696, P=0.000),联合检测与病理检查的一致性良好(Kappa值=0.926, P=0.000)。结论 TCT联合HR-HPV检测在宫颈癌及癌前病变早期筛查中的应用价值较高,能够有效提高诊断准确度,为临床明确病情提供可靠依据。
Objective To explore the application value of thinprep cytologic test(TCT) combined with high-risk human papillomavirus(HR-HPV) detection in the early screening of cervical cancer and precancerous lesions. Methods 181 women who underwent screening for cervical cancer and precancerous lesions in our hospital from January 2019 to December 2020 were selected, and all cases underwent TCT and HR-HPV examinations. Using pathological results as the "gold standard" for diagnosis, the value of single detection and combined detection of TCT and HR-HPV in disease screening and the consistency with pathological results were analyzed.Results The sensitivity, specificity, accuracy and negative predictive value of the combined detection of TCT and HR-HPV were higher than those of single detections of TCT and HR-HPV in the screening of cervical cancer and precancerous lesions(P <0.05). Kappa test showed that the consistency of TCT and HR-HPV with pathological examination were acceptable(Kappa value = 0.527, P = 0.000;Kappa value =0.696, P = 0.000), and the consistency of combined detection with pathological examination was good(Kappa value = 0.926, P = 0.000).Conclusions TCT combined with HR-HPV detection has high application value in the early screening of cervical cancer and precancerous lesions, which can effectively improve the diagnostic accuracy and provide a reliable basis for clinical diagnosis of the disease.
