摘要
目的研究内镜下钛夹治疗儿童结直肠大息肉(≥2.0 cm)的临床疗效及安全性。方法选择2014年12月至2020年6月郑州大学第三附属医院收治的86例结直肠大息肉患儿为研究组,另选同期随机22例未使用钛夹患儿为对照组。回顾性分析每组息肉大小、位置、术后出血率、感染及腹痛等并发症发生率。结果所有息肉切除均获得成功,两组年龄(P=0.831)、性别(P=0.339)、息肉大小(P=0.699)、息肉位置(P=0.654)比较差异均无统计学意义;研究组迟发性出血发生率明显低于对照组(3.5%、22.7%,P=0.009),总体并发症发生率也低于对照组(8.1%、27.3%,P=0.036),均未发生肠穿孔等严重并发症。结论在≥2.0 cm儿童结直肠息肉中,内镜下联合钛夹治疗可以预防术后延迟出血,降低术后并发症,疗效确切、安全可行。
【Objective】To investigate the clinical efficacy and safety of endoscopic resection combined with endoclips in the treatment of colonic polyps in children.【Methods】From December 2014 to June 2020,large colorectal polyps(diameter≥2.0 cm)were eligible for inclusion and were retrospectively analyzed.Study group:86 patients with large colorectal polyps treated with endoclips;Control group:22 cases who did not use endoclips during the same period.The primary outcome were the size,location,endoclips,postoperative bleeding rate,infection and abdominal pain in each group.【Results】All polyps were resected successfully,and there were no statistical differences in age,gender,size and location of polyps between the two groups(P>0.05).The incidence of postpolypectomy bleeding in the study group was lower than that in the control group(3.5%vs.22.7%,P=0.009),and the overall incidence of complications was also lower(8.1%vs.27.3%,P=0.036).No case of intestinal perforation occurred.【Conclusion】In children with colorectal polyps≥2.0 cm,combined treatment with endoclips can reduce postoperative bleeding and complications.It is effective,safe and feasible,and is the first choice for the treatment of large intestinal polyps in children.
