摘要
目的:探讨社区化管理在居家腹膜透析患者中的应用效果。方法:以2019年1月1日在复旦大学附属金山医院肾内科接受腹膜透析,且符合纳入和排除标准的所有患者为研究对象,共48人,平均年龄(63.5±12.79)岁,其中男性22人,女性26人。管理由符合资质的社区医师统一进行。以2019年全年实施常规管理为社区化管理前,以2020年全年实施社区化管理为社区化管理后。比较腹膜透析患者社区化管理前后的操作合格率、腹透管出口处评分、腹膜炎发生率、生活质量评分、水肿情况、血压控制达标率等指标。结果:社区化管理后,患者操作合格率显著升高,腹透管出口处评分、腹膜炎发生率显著下降,水肿严重程度显著改善,生活质量评分显著下降,高血压控制率显著提高。结论:社区化管理有助于改善居家腹透患者腹透操作技能、改善并发症的发生情况,提高生活质量。
Objective:To explore the application effect of community management for patients with peritoneal dialysis at home.Methods:All patients who received peritoneal dialysis in the Department of Nephrology of Jinshan Hospital Affiliated to Fudan University on January 1,2019 and met the inclusion and exclusion criteria were selected as the research subjects,a total of 48 patients.The average age was(63.5±12.79)years,including 22 males and 26 females.Management was carried out uniformly by qualified community physicians.The implementation of conventional management throughout the year in 2019 was considered as before community management,and the implementation of community management throughout the year in 2020 was considered as after community management.The operation qualification rate,outlet score of peritoneal dialysis tube,incidence of peritonitis,quality of life score,edema,and blood pressure control rate of peritoneal dialysis patients before and after community management were compared.Results:After the community management,the operation qualification rate of patients was significantly increased,and the score at the outlet of the peritoneal dialysis tube and the incidence of peritonitis were significantly decreased,The severity of edema was significantly improved,the score of quality of life was significantly decreased,and the control rate of hypertension was significantly improved.Conclusion:Community-based management is helpful to improve the peritoneal dialysis operation skills,the incidence of complications,and the quality of life of peritoneal dialysis patients at home.
