摘要
目的对比腹腔镜子宫肌瘤剔除术与经腹子宫肌瘤剔除术在子宫肌瘤中的作用。方法选取2019年1月—2020年12月江苏省东台市人民医院妇产科收治的子宫肌瘤患者80例,经奇偶数法平均分配,40例奇数者为对照组,用经腹子宫肌瘤剔除术,40例偶数者为观察组,用腹腔镜子宫肌瘤剔除术,对比炎症因子、应激指标、血管生成指标和并发症。结果两组患者术前炎症因子、应激指标和血管生成因子水平对比,差异无统计学意义(P>0.05);观察组术后C反应蛋白为(20.18±2.14)mg/L,白细胞介素-6为(32.54±8.16)ng/mL,皮质醇、多巴胺和肾上腺素分别为(118.21±10.45)ng/mL、(552.14±17.68)mmol/24 h和(342.16±18.54)ng/L低于对照组,白细胞介素-2水平高于对照组,差异有统计学意义(t=17.051、8.781、8.743、29.303、5.970、4.745,P<0.05);两组治疗后血管生成指标对比,差异有统计学意义(P<0.05)。结论与经腹子宫肌瘤剔除术比较,予以子宫肌瘤患者腹腔镜子宫肌瘤剔除术,可调节炎症指标,减少应激反应,对血管生成指标无明显影响,安全性广。
Objective To compare the effect of laparoscopic myomectomy and transabdominal myomectomy in uterine fibroids.Methods From January 2019 to December 2020,80 patients with uterine fibroids were selected from the Department of Obstetrics and Gynecology,Dongtai People's Hospital,Jiangsu Province.The odd-numbered cases were divided equally,40 cases with odd number were the control group,and tronsabdominal 40 cases with even number were the observation group,and treated with laparoscopic myomectomy.Inflammatory factors,stress indicators,angiogenesis indicators and complications were compared.Results Comparison of the levels of preoperative inflammatory factors,stress indicators and angiogenesis factors between the two groups,the difference was not statistically significant(P>0.05).The postoperative C-reactive protein in the observation group was(20.18±2.14)mg/L,and the interleukin-6 was(32.54±8.16)ng/mL.Cortisol,dopamine and epinephrine were(118.21±10.45)ng/mL,(552.14±17.68)mmol/24 h and(342.16±18.54)ng/L,respectively,lower than the control group,the level of interleukin-2 was higher than that in the control group,the difference was statistically significant(t=17.051,8.781,8.743,29.303,5.970,4.745,P<0.05).There was statistically significant differences in angiogenesis indexes between the two groups after treatment(P<0.05).Conclusion Compared with transabdominal myomectomy,laparoscopic myomectomy in patients with uterine fibroids can regulate inflammatory indexes and reduce stress response.It has no significant effect on angiogenesis indexes,has wide safety.
