呼吸机与PS联合治疗新生儿重症MAS前后OI的变化研究

摘要 目的探究呼吸机与肺表面活性物质(PS)联合治疗新生儿吸入综合征(MAS)的临床效果。方法选择2018年2月—2020年4月在该院接受治疗的102例患儿,随机投掷法分为对照组(行呼吸机治疗)与观察组(行呼吸机与PS联合治疗),每组51例。比较两组氧合指数、血气分析指标、呼吸机参数、呼吸机上机、住院时间。结果经治疗后,观察组治疗后12 hOI(13.11±1.75)、24 h(12.04±1.37)、36 h(11.12±1.13)%均低于对照组,差异有统计学意义(t=12.782、11.775、14.698,P<0.05);治疗后,观察组PaCO_(2)、PIP、MAP、FiO_(2)低于对照组,PaO_(2)高于对照组,差异有统计学意义(P<0.05);观察组呼吸机上机时间、住院时间均低于对照组,差异有统计学意义(P<0.05)。结论呼吸机联合PS治疗该疾病效果显著。 Objective To explore the clinical effect of ventilator combined with pulmonary surfactant(PS)in the treatment of neonatal aspiration syndrome(MAS).Methods 102 children were selected from February 2018 to April 2020.They were randomly divided into the control group(with ventilator treatment)and the observation group(with ventilator and PS combined treatment),with 51 cases in each group.The oxygenation index,blood gas analysis indexes,ventilator parameters,ventilator on-machine and hospitalization time were compared between the two groups.Results After treatment,the OI of the observation group at 12 h(13.11±1.75)%,24 h(12.04±1.37)%and 36 h(11.12±1.13)%after treatment were all lower than those in the control group,the difference was statistically significant(t=12.782,11.775,14.698,P<0.05).After treatment,PaCO_(2),PIP,MAP,FiO_(2) in the observation group were lower than those in the control group,and PaO_(2) was higher than that in the control group,with statistical significance(P<0.05).The ventilator-on time and hospitalization time of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).Conclusion Ventilator combined with PS has a significant effect in the treatment of this disease.
