摘要
目的:观察结核感染T细胞斑点试验(T-SPOT.TB)联合痰涂片抗酸染色技术检测在肺结核诊断中的效能。方法:选取118例疑似肺结核患者为研究对象,所有患者均接受T-SPOT.TB和痰涂片抗酸染色技术检测,以结核分枝杆菌痰培养结果为金标准,比较T-SPOT.TB、痰涂片抗酸染色技术单项及联合检测在肺结核诊断中的效能。结果:经结核分枝杆菌痰培养确诊肺结核92例,非肺结核26例;T-SPOT.TB检测结果显示阳性90例,阴性28例;痰涂片抗酸染色技术检测结果显示阳性61例,阴性57例;联合检测结果显示阳性92例,阴性26例;T-SPOT.TB、痰涂片抗酸染色技术单项检测与联合检测诊断肺结核的特异度比较,差异均无统计学意义(P>0.05);T-SPOT.TB检测诊断肺结核的灵敏度、准确度均高于痰涂片抗酸染色技术检测,T-SPOT.TB联合痰涂片抗酸染色技术检测诊断肺结核的灵敏度、准确度均高于两者单项检测,差异有统计学意义(P<0.05)。结论:T-SPOT.TB联合痰涂片抗酸染色技术检测诊断肺结核的效能高于两者单项检测。
Objective: To observe efficacy of T-cell spot test for tuberculosis infection(T-SPOT.TB) combined with sputum smear acidfast staining technique in diagnosis of pulmonary tuberculosis. Methods: 118 patients with suspected pulmonary tuberculosis were selected as the research subjects, and all of them were tested by T-SPOT.TB and sputum smear acid-fast staining techniques. Taking the results of Mycobacterium tuberculosis sputum culture as the gold standard, the efficacies of T-SPOT.TB, acid-fast staining technique of sputum smear and combined detection in the diagnosis of pulmonary tuberculosis were compared. Results: The results of the Mycobacterium tuberculosis sputum culture confirmed 92 cases were diagnosed as pulmonary tuberculosis and 26 cases were not suffered from non-pulmonary tuberculosis. The T-SPOT.TB test results showed that 90 cases were positive and 28 were negative. The results of acid-fast staining of sputum smear showed 61 cases were positive and 57 were negative. The combined test results showed there were 92 positive cases and 26 negative cases. There were no significant differences in the specificity among single T-SPOT.TB, single sputum smear acid-fast staining technique and the combined detection(P>0.05). The sensitivity and accuracy of T-SPOT.TB detection in the diagnosis of pulmonary tuberculosis were higher than those of sputum smear acid-fast staining technique;the sensitivity and accuracy of T-SPOT.TB combined with sputum smear acid-fast staining technique were higher than two single tests;and the differences were statistically significant(P<0.05). Conclusions: The detection efficiency of T-SPOT.TB combined with acid-fast staining of sputum smear in the diagnosis of pulmonary tuberculosis is higher than the two single tests.
