摘要
目的:探讨急性心肌梗死后恶性室性心律失常使用胺碘酮和利多卡因治疗的效果。方法:选取2018年1月-2019年6月龙岩市第二医院心内科80例急性心肌梗死后恶性室性心律失常患者。采用计算机随机抽样分为利多卡因组(n=40)和胺碘酮组(n=40)。所有患者入院后均根据最新的急性心肌梗死诊治指南进行治疗,利多卡因组实施利多卡因辅助治疗,胺碘酮组实施胺碘酮辅助治疗。比较两组临床疗效,治疗前后脑利尿钠肽(BNP),不良反应。结果:胺碘酮组总有效率高于利多卡因组(P<0.05)。两组治疗前BNP水平比较差异无统计学意义(P>0.05),胺碘酮组治疗后BNP水平低于利多卡因组(P<0.05)。胺碘酮组不良反应发生率低于利多卡因组(P<0.05)。结论:对比利多卡因,急性心肌梗死后恶性室性心律失常患者实施胺碘酮治疗可更有效地控制心律失常的病情、改善心功能、且用药安全性良好。
Objective:To investigate the effect of Amiodarone and Lidocaine in patients with malignant ventricular arrhythmia after acute myocardial infarction.Method:A total of 80 patients with malignant ventricular arrhythmia after acute myocardial infarction were selected from the department of cardiology of Longyan Second Hospital from January 2018 to June 2019.The patients were divided into Lidocaine group(n=40)and Amiodarone group(n=40)by computer random sampling.After admission,all patients were treated according to the latest guidelines for the diagnosis and treatment of acute myocardial infarction.Lidocaine group received Lidocaine adjuvant therapy,and Amiodarone group received Amiodarone adjuvant therapy.The clinical efficacy,brain natriuretic peptide(BNP)before and after treatment and adverse reactions were compared between the two groups.Result:The total effective rate of Amiodarone group was higher than that of Lidocaine group(P<0.05).There was no significant difference in BNP level between the two groups before treatment(P>0.05).BNP level in Amiodarone group was lower than that in Lidocaine group after treatment(P<0.05).The incidence of adverse reactions in Amiodarone group was lower than that in Lidocaine group(P<0.05).Conclusion:Compared with Lidocaine,Amiodarone treatment for patients with malignant ventricular arrhythmia after acute myocardial infarction can control the condition of arrhythmia more effectively,improve cardiac function,and with good drug safety.
