摘要
目的分析清肺汤联合机械通气对急性肺损伤(ALI)合并急性呼吸窘迫综合征(ARDS)患儿肺泡巨噬细胞(AM)、肿瘤坏死因子(TNF-α)及血清炎症因子的影响。方法选取医院2019年12月—2020年3月收治的102例ALI合并ARDS患儿作为研究对象,按照数字表法分为两组,将采用机械通气等常规干预的51例患儿设为对照组,在常规干预的基础上采取基于清肺汤的综合治疗策略的51例患儿设为观察组。分析两种干预方式对患儿临床疗效的影响。结果观察组治疗后血清细胞间细胞间黏附分子-1(ICAM-1)及TNF-α表达水平明显低于对照组,对比差异均有统计学意义(P<0.05)。治疗后观察组促炎性介质表达水平脂多糖结合蛋白(LBP)、IL-6显著优于对照组,差异较大(P<0.05)。两组干预前肺功能指标差异无统计学意义(P>0.05)。两组干预前肺功能指标差异无统计学意义(P>0.05)。干预后观察组肺功能指标肺活量(VC)及第1秒用力呼气量(FEV;)明显高于对照组,差异显著(P<0.05)。结论清肺汤鼻饲治疗联合机械通气的综合治疗策略可有效改善ALI合并ARDS患儿血清炎症因子表达水平及肺功能,值得临床关注。
Objective To analyze the effects of Qingfei Decoction(清肺汤)combined with mechanical ventilation on AM,TNF-αand serum inflammatory factors in children with ARDS complicated by ALI.Methods A total of 102 children with ARDS admitted to our hospital from December 2019 to March 2020 were selected as the research objects,and divided into two groups according to the numerical table method.51 children with routine intervention such as mechanical ventilation were set as the control group,and 51 children with comprehensive treatment strategy based on Qingfei Decoction on the basis of routine intervention were set as the observation group.The influence of two intervention Methods on the clinical efficacy of two groups were analyzed.Results After treatment,the expression levels of ICAM-1 and TNF-αin serum of the observation group were significantly lower than those of the control group(P<0.05).After treatment,the expression levels of pro-inflammatory mediators LBP and IL-6 in observation group were significantly better than those in control group(P<0.05).There was no significant difference in pulmonary function indexes between the two groups before intervention(P>0.05).There was no significant difference in pulmonary function indexes between the two groups before intervention(P>0.05).The pulmonary function indexes VC and FEV1 in the observation group were significantly higher than those in the control group after intervention(P<0.05).Conclusion Qingfei Decoction nasal feeding combined with mechanical ventilation can effectively improve the expression levels of serum inflammatory factors and lung function in children with ALI complicated with ARDS,worthy of clinical attention.
