摘要
目的探讨ALARA原则在13~19岁肺结核患者低剂量螺旋CT检查中的应用。方法回顾性分析2019年4月至2021年4月在鹤壁煤业(集团)有限责任公司总医院影像科行CT筛查肺结核的86例体检者,依照检查方式分为参照组(n=43,常规剂量CT照射)和试验组(n=43,依照体质量确定CT照射剂量)。比较两组肺部CT扫描成像质量。结果与参照组相比,试验组有效剂量(ED)、剂量长度乘积(DLP)、CT剂量指数(CTDI)较低(P<0.05);试验组肺窗、纵隔窗及图像总体质量评分高于参照组(P<0.05),试验组图像质量评分均≥3分,满足临床确诊成像质量要求。结论对13~19岁肺结核患者运用ALARA原则,既可满足临床诊疗需要,也可降低CT检查辐射剂量,可推广。
Objective To explore the application of ALARA principle in low-dose spiral CT examination of tuberculosis patients aged 13-19 years.Methods A total of 86 patients who underwent CT screening for tuberculosis in the Department of Radiology from April 2019 to April 2021 in the General Hospital of Hebi Coal Industry(Group)Co.,Ltd were retrospectively analyzed.According to examination methods,they were divided into reference group(n=43,conventional dose of CT irradiation)and experimental group(n=43,CT irradiation dose was determined according to different body weight).The imaging quality of lung CT scan was compared between the two groups.Results Compared with the reference group,the efective dose(ED),dose length product(DLP)and CT dose index(CTDI)of the experimental group were lower(P<0.05).The scores of lung window,mediastinal window and overall image quality in the experimental group were higher than those in the reference group(P<0.05).The scores of image quality in the experimental group were all≥3 points,which met the requirements of clinical imaging quality.Conclusion For 13-19 years old tuberculosis patients,the application of ALARA principle can not only meet the needs of clinical diagnosis and treatment,but also reduce the radiation dose of CT examination,and can be popularized.
