荧光染色法和传统KOH湿片法在甲真菌病诊断中的应用比较 被引量:6

Fluorescent staining method vs KOH wet mount method in the diagnosis of finger(toe)nail infection

摘要 目的 探讨真菌荧光染色法与传统KOH湿片法在临床甲真菌镜检中的应用效果。方法 选择2017年6月至2018年4月我院600例皮肤科门诊就诊疑似真菌感染患者指(趾)甲镜检标本,分别运用荧光染色法和KOH湿片法对这些标本进行检测,记录实验数据,并对检测结果进行比较。结果 荧光染色法检测阳性为418例,阳性率为70%,其中指甲阳性130例,趾甲阳性288例。KOH湿片法检测阳性为354例,阳性率为59%,其中指甲97例,趾甲257例。指甲(χ^(2)=8.816,P=0.003)、趾甲(χ^(2)=7.884,P=0.005)及总阳性率(χ^(2)=14.876,P<0.001)在两种方法中差异均有统计学意义。结论 在对指(趾)甲真菌感染的诊断中,荧光染色法的阳性检出率明显高于KOH湿片法,是一种快速、准确的真菌镜检方法,值得临床推广使用。 Objective To compare the fluorescent staining method and KOH wet mount method in the diagnosis of finger(toe) nail infection. Methods Finger(toe) nail microscopy specimens from 600 patients with suspected fungal infection in our hospital from Jun 2017 to Apr 2018 were collected and detected with fluorescent staining and KOH wet mount methods, respectively. The data were recorded and compared. Results There were 418 positive cases by fluorescent staining method;the positive rate was 70%. Among them, there were 130 cases of positive nails and 288 cases of positive toenails. KOH wet mount resulted in 354 positive cases, including 97 cases of positive nails and 257 cases of positive toenails;the positive rate was 59%. There were significant differences in the positive rates of nails(χ^(2)=8.816, P=0.003), toenails(χ^(2)=7.884, P=0.005) and the total positive rate(χ^(2)=14.876, P<0.001) between the two methods. Conclusion In the diagnosis of onychomycosis infection, the positive detection rate by fluorescent staining method was significantly higher than that by KOH wet mount method, which was a fast and accurate method for fungal microscopy, and can be widely used clinically.
