摘要
目的观察补肾益肺汤治疗慢性阻塞性肺疾病(COPD)稳定期肺肾气虚证患者的疗效及对肺功能和免疫功能的影响。方法将100例COPD稳定期肺肾气虚证患者按照随机数字表法分为2组。对照组50例予常规西药联合肺康复治疗,治疗组50例在对照组治疗基础上加补肾益肺汤治疗,2组均治疗1个月。比较2组临床疗效,以及2组治疗前后中医证候评分、肺功能[最大呼气峰流速(PEF)、肺活量(FVC)、第1 s用力呼气容积(FEV_(1))、FEV_(1)/FVC]、6 min步行距离(6MWD)和T淋巴细胞亚群CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)水平。结果治疗组总有效率96.00%(48/50),对照组总有效率80.00%(40/50),治疗组总有效率高于对照组(P<0.05)。治疗后2组中医证候各项评分及主症、次症总分均较本组治疗前下降(P<0.05),且治疗组均低于对照组(P<0.05)。治疗后2组PEF、FVC、FEV_(1)、FEV_(1)/FVC、6MWD、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均较本组治疗前提高(P<0.05),且治疗组均高于对照组(P<0.05)。结论在常规西药联合肺康复治疗基础上加补肾益肺汤治疗COPD稳定期肺肾气虚证,能明显提高临床疗效,减轻临床症状,提高患者肺功能和免疫功能。
Objective To observe the clinical efficacy of Bushen Yifei Decoction for patients with stable chronic obstructive pulmonary disease(COPD)of lung-kidney qi deficiency syndrome and how to impact on pulmonary function and immune function.Methods Totally 50 COPD patients(lung-kidney qi deficiency syndrome)were randomly assigned into two equal groups,patients in control group were treated with conventional Western medicine combined with pulmonary rehabilitation,and patients in treatment group with Bushen Yifei Decoction based on control group additionally.The treatment period was 1 month,traditional Chinese medicine(TCM)syndrome score,pulmonary function[peak expiratory flow(PEF),forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1)),FEV_(1)/FVC],6-Minute Walking Distance(6-MWD),T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+))were included as comparators;the curative effect was counted.Results The overall effective rate in treatment group and control group was 96.00%(48/50)and 80.00%(40/50),respectively,the curative effect being better in treatment group than control group(P<0.05).After treatment,decreased TCM syndromes(main and secondary symptoms)scores in groups were found(P<0.05),which were decreased in treatment group as comparison with control group(P<0.05);increased PEF,FVC,FEV_(1),FEV_(1)/FVC,6MWD,CD3^(+),CD4^(+),CD4^(+)/CD8^(+) were recorded(P<0.05),improvement of which in treatment group were significantly better(P<0.05).Conclusion For stable COPD patients(lung-kidney qi deficiency syndrome),administration of Bushen Yifei Decoction is obviously capable of improving clinical efficacy,pulmonary and immune function,relieving clinical symptoms.
