椎-基底动脉供血不足诊断的应用进展 被引量:3

摘要 椎-基底脉动脉供血不足(VBI)是一种临床常见病和多发病。在现代医学研究中,国外研究者大多以“后循环缺血(PCI)”来称呼VBI,但在我国仍以“VBI”广泛应用在临床和学术交流中。VBI的多发人群为50岁以上人群,近年来逐渐年轻化。椎-基底动脉由于走向曲折、变异复杂,病症辨识难度增加,漏诊情况时有发生。目前,数字减影心血管造影术(DSA)、多层螺旋CT血管成像(MSCTA)、磁共振血管造影(magnetic resonance angiography,MRA)、经颅多普勒超声(TCD)等多种诊断方法已应用于临床,其中TCD检查具有无创、灵敏度及可重复性高的特点,能够清晰显示血管的实际情况,已成为临床诊断血管疾病的重要手段。本研究通过总结VBI的发病机制及临床表现,综述VBI常用的诊断方法,并重点分析TCD在VBI诊断中的作用,旨在为相关研究提供参考。 Vertebro-basilar artery insufficiency(VBI)is a common and frequentlyoccurring clinical disease.In modern medical research,most foreign researchers call VBI by"posterior circulation ischemia(PCI)",but"VBI"is still widely used in clinical and academic exchanges in China.People over 50 years old encountered VBI more easily,while the age of whom suffering this disease has become younger in recent years.Because of the zigzag and complex variation of vertebro-basilar artery,it is difficult to identify the symptoms,resulting in misdiagnosis on occasion.At present,digital subtraction angiography(DSA),multislice spiral CT angiography(MSCTA),magnetic resonance angiography(MRA),transcranial Doppler ultrasound(TCD)and other diagnostic methods have been applied in clinical practice.Among them,TCD examination has the characteristics of non-invasive,high-sensitivity and high-repeatability,which can clearly show the actual situation of blood vessels and has become an important means of clinical diagnosis of vascular diseases.By summarizing the pathogenesis and clinical manifestations of VBI,the common examination methods of VBI were introduced,and the role of TCD in the examination of VBI was analyzed emphatically in this paper,aiming to provide reference for related studies.
