摘要
目的探讨运动平板试验联合颈动脉血管彩超在飞行员筛查中的应用效果及效能ROC曲线。方法选择2017年3月-2021年4月民航空勤人员438例作为对象,均进行运动平板试验、颈动脉血管彩超检查,以冠脉造影或冠脉CTA检查结果作为诊断冠脉狭窄的"标准",分析不同检查方法在冠脉狭窄筛查中的应用效果;绘制ROC曲线,分析运动平板试验联合颈动脉血管彩超在飞行员冠脉狭窄筛查中的筛查效能。结果 438例飞行人员中冠脉造影或冠脉CTA检查最终确诊冠脉狭窄65例。运动平板试验检查与标准诊断符合率为92.69%(406/438);诊断灵敏度为86.15%(56/65),特异度为93.83%(350/373);颈动脉血管彩超检查与标准诊断符合率为92.92%(407/438);诊断灵敏度为87.69%(57/65),特异度为92.76%(346/373);运动平板试验联合颈动脉血管彩超检查与标准诊断符合率为97.03%(425/438);诊断灵敏度为95.38%(62/65),特异度为97.31%(363/373);ROC曲线结果表明:运动平板试验联合颈动脉血管彩超在飞行员冠脉狭窄筛查灵敏度及特异度均高于单一运动平板试验、颈动脉血管彩超(P<0.05)。结论运动平板试验及颈动脉血管彩超用于飞行员筛查中能提高冠脉狭窄检出率,且两种方法联合应用可提高筛查灵敏度与特异度。
Objective To explore the effect of exercise treadmill test combined with carotid artery color Doppler ultrasound in pilot screening and the ROC curve. Methods 438 civil aviation crews from March 2017 to April 2021 were selected as subjects. All of them underwent exercise treadmill test and carotid artery color Doppler ultrasound examination. The results of coronary angiography or coronary artery CTA examination were used as the “gold standard”, and the differences were analyzed. The application effect of the inspection method in the screening of coronary artery stenosis;draw ROC curve, analyze the screening efficiency of exercise treadmill test combined with carotid artery color Doppler ultrasound in pilot screening. Results Among 438 pilots, 65 cases of coronary artery stenosis were finally diagnosed by coronary angiography or coronary CTA. The compliance rate of exercise treadmill test and gold standard diagnosis was 92.69%(406/438);the diagnostic sensitivity was 86.15%(56/65), and the specificity was 93.83%(350/373);carotid artery color Doppler ultrasound examination and gold standard diagnosis The coincidence rate was 92.92%(407/438);the diagnostic sensitivity was 87.69%(57/65), and the specificity was 92.92%(407/438);the coincidence rate of exercise treadmill test combined with carotid artery color Doppler ultrasound examination and gold standard diagnosis was 97.03%(425/438);the diagnostic sensitivity is 95.38%(62/65), and the specificity is 97.31%(363/373);ROC curve results show that: exercise treadmill test combined with carotid artery color Doppler ultrasound in pilot screening sensitivity and specificity Both were higher than the single exercise treadmill test and carotid artery color Doppler ultrasound(P<0.05). Conclusions Exercise treadmill test and carotid artery color Doppler ultrasound can improve the detection rate of coronary stenosis in pilot screening, and the combined application of the two methods can improve the sensitivity and specificity of screening.
