摘要
目的筛选共识度较高且较为规范的中风后失语中药处方,旨在规范中风后失语的中药治疗,进一步提升其临床疗效。方法采用回顾性研究方法,收集82例中风后失语患者的有效中药处方,运用SPSS、Gephi软件进行复杂网络分析,提炼初步的方药组成,然后应用德尔菲法问卷调查,对该处方进行论证和优化,筛选得到最终的中药处方。结果经两轮德尔菲法问卷调查,专家代表性、积极性及权威性均较高,专家意见集中程度及协调性较好,共筛选出17味中药,构成最终的中药处方,该处方主要针对本病“肾虚痰瘀闭窍”的核心病机,重在“益髓醒神、活血通络、化痰开窍”。结论应用德尔菲法筛选出的中药处方共识度较高,结果较为真实可靠,在一定程度上能够为治疗中风后失语的核心中药发现和新药研发提供新的思路和方法。
Objective To screen the prescriptions of traditional Chinese medicine for post-stroke aphasia(PSA)with a high degree of consensus and standardization,aiming to standardize the Chinese medicine treatment of PSA and to further improve its clinical efficacy.Methods A retrospective research method was used to collect the effective Chinese medicine prescriptions of 82 patients with PSA.We used SPSS and Gephi software to perform complex network analysis,and to extract the preliminary Chinese prescription composition,then to apply Delphi questionnaire survey to demonstrate and optimize the prescription,finally screening to get the Chinese medicine prescription.Results After two rounds of Delphi questionnaire surveys,the levels of representativeness,enthusiasm and authority of experts were quite high,and the concentration and coordination of experts opinions were good.A total of 17 Chinese herbs were screened out to form the final Chinese medicine prescription.The targets of prescriptions were mainly at the core pathogenesis“Kidney deficiency,phlegm and blood stasis and orifice closure”of PSA,and focused on“nourishing the marrow and refreshing the mind,invigorating blood and dredging collaterals,and resolving phlegm and resuscitation”.Conclusion The core Chinese medicine prescriptions screened by the Delphi method have a high degree of consensus,and the results are more authentic and reliable.To a certain extent,this study can provide new ideas and methods for the discovery of core Chinese medicines and new drug development for the treatment of PSA.
