摘要
目的研究急性脑梗死伴2型糖尿病患者血小板miR-223表达水平与阿司匹林抵抗的相关性。方法回顾性选取2018年12月至2019年12月河北北方学院附属第一医院急性脑梗死患者120例,根据是否合并2型糖尿病分为合并组、未合并组两组,各60例。统计分析两组患者的血小板指标、血小板最大聚集率(MAR)、阿司匹林抵抗发生情况、miR-223表达水平;比较阿司匹林抵抗与阿司匹林敏感患者的miR-223表达水平。结果两组患者治疗后的血小板压积、血小板平均体积、血小板计数、血小板分布宽度均低于治疗前,差异均有统计学意义(P<0.05);治疗前,两组患者的血小板压积、血小板平均体积、血小板计数、血小板分布宽度之间的差异均无统计学意义(P>0.05);治疗后,合并组患者的血小板压积、血小板平均体积、血小板计数、血小板分布宽度为(0.15±0.03)%、(7.33±1.22)fL、(121.22±9.22)×10^(9)/L、(15.33±2.44)%,均低于未合并组[(0.21±0.11)%、(8.33±1.13)fL、(226.33±9.20)×10^(9)/L、(17.03±2.13)%],差异均有统计学意义(P<0.05)。合并组患者的诱导剂为二磷酸腺苷、花生四烯酸时MAR为(76.88±6.74)%、(68.55±9.55)%,高于未合并组[(71.22±8.66)%、(51.22±9.22)%],阿司匹林抵抗发生率为33.33%,高于未合并组(16.67%),阿司匹林敏感发生率为66.67%,低于未合并组(83.33%),miR-223表达水平为1.33±0.24,高于未合并组(0.71±0.16),差异有统计学意义(P<0.05)。阿司匹林抵抗患者的miR-223表达水平为2.41±0.22,明显高于阿司匹林敏感组(0.98±0.17),差异有统计学意义(P<0.05)。结论急性脑梗死伴2型糖尿病患者血小板miR-223表达水平升高可能与阿司匹林抵抗相关。
Objective To study the correlation between platelet miR-223 and aspirin resistance in patients with acute cerebral infarction complicated with type 2 diabetes mellitus.Methods A total of 120 patients with acute cerebral infarction in the First Affiliated Hospital of Hebei North University from December 2018 to December 2019 were retrospectively selected and divided into the combined group and the non-combined group according to the conditions of type 2 diabetes,with 60 patients in each group.The platelet indexes,maximum platelet aggregation rate(MAR),occurrence of aspirin resistance and level of miR-223 expression in the two groups were compared.The expression levels of miR-223 in aspirin resistant and aspirin sensitive patients were statistically analyzed.Results After treatment,the platelet hematocrit,mean platelet volume,platelet count and platelet distribution width in 2 groups were lower than before(P<0.05);before treatment,there were no significant differences in platelet hematocrit,mean platelet volume,platelet count,and platelet distribution width between 2 groups(P<0.05);after treatment,the platelet hematocrit,mean platelet volume,platelet count and platelet distribution width of patients in the combined group were(0.15±0.03)%,(7.33±1.22)fL,(121.22±9.22)×10^(9)/L,(15.33±2.44)%,which were lower than those in the uncombined group[(0.21±0.11)%,(8.33±1.13)fL,(226.33±9.20)×10^(9)/L,(17.03±2.13)%],the differences were statistically significant(P<0.05).The MARS in the combined group were(76.88±6.74)%,(68.55±9.55)%,which were higher than those in the uncombined group[(71.22±8.66)%,(51.22±9.22)%].When the inducers were adenosine diphosphate and chemotetraenic acid,the differences were statistically significant(P<0.05),the incidence of aspirin resistance was 33.33%,which was higher than that in the uncombined group(16.67%),the incidence of aspirin sensitivity was 66.67%,which was lower than that in the uncombined group(83.33%),and the level of miR-223 expression was 1.33±0.24,which was higher than that in the uncombined group(0.71±0.16),and the differences were statistically significant(P<0.05).The expression level of miR-223 in aspirin-resistant patients was 2.41±0.22,which was significantly higher than that in aspirin-sensitive group(0.98±0.17),and the difference was statistically significant(P<0.05).Conclusion The elevated expression of miR-223 on platelets in acute cerebral infarction patients with type 2 diabetes mellitus may be related to aspirin resistance.
