摘要
目的探讨大剂量盐酸氨溴索对改善住院百日咳患儿痉挛性咳嗽症状的临床疗效。方法由汕头大学医学院第二附属医院牵头,联合广东省内另16家单位开展多中心、开放性、前瞻性、非随机对照临床研究。将2019年1月至2020年4月期间确诊的百日咳住院患儿分为对照组、盐酸氨溴索常规剂量组(7.5 mg/次,静脉滴注,2次/d)和盐酸氨溴索大剂量组(每次10 mg/kg,静脉滴注,2次/d,即20 mg·kg^(-1)·d^(-1))。记录治疗前、治疗第1天、第3天、第7天、第10天咳嗽症状积分,对比分析。结果共240例患儿纳入研究,212例完成研究,其中对照组27例,常规剂量组96例,大剂量组89例。3组咳嗽症状积分均值在治疗前分别为9.4、9.5、10.0分,治疗第3天分别为8.3、7.6、6.8分,第7天分别为7.0、5.9、4.8分,第10天分别为4.2、2.9、2.5分。根据统计结果随着治疗时间的延长,对照组、常规剂量组与大剂量组的咳嗽积分下降幅度不同,以大剂量组的咳嗽积分下降幅度最大。对比对照组、常规剂量组和大剂量组的痉挛性咳嗽症状积分减分率,在第3天、第7天、第10天,3组间差异有统计学意义(P值均<0.05),其中大剂量组咳嗽症状积分的减分率明显高于对照组和常规剂量组(P值均<0.05),而常规剂量组与对照组比较差异无统计学意义。结论大剂量盐酸氨溴索(20 mg·kg^(-1)·d^(-1))可显著改善儿童百日咳痉挛性咳嗽症状。
Objective To explore the clinical effect of high-dose ambroxol hydrochloride(AH)on improving the symptoms of spastic cough in hospitalized children with whooping cough.Methods A multi-center,open,prospective,non-randomized controlled clinical study carried out by the union of 17 units in Guangdong Province,initiated by the Second Affiliated Hospital of Shantou University Medical College.The hospitalized children with whooping cough diagnosed from January 2019 to April 2020 were divided into a control group,a AH regular-dose group(7.5 mg each time,twice a day,by intravenous infusion),and a AH high-dose group(20 mg·kg^(-1)·d^(-1),by intravenous infusion).The cough symptom scores were recorded and analyzed comparatively on days 0(pre-injection),1,3,7 and 10.Results Totally 212 cases had completed the research in included 240 children,those cases were divided into control group(n=27),regular-dose group(n=96)and high-dose group(n=89).The average scores of cough symptoms in the above three groups in turn were 9.4,9.5,10.0;and 8.3,7.6,6.8;and 7.0,5.9,4.8;and 4.2,2.9,2.5 on days 0,3,7,10,respectively.The decrease range of cough symptom scores in three groups were different as treatment time extension,while was most common in high-dose group.In comparison of three groups,the difference of spastic cough scores on days 3,7,10 was statistically significant(P<0.01),which in high-dose group was significantly higher than control group and regular-dose group,with statistically significant(P<0.05),while no difference between regular-dose group and control group was not statistically significant.Conclusions High-dose AH(20 mg·kg^(-1)·d^(-1))is capable of improving the symptoms of spastic cough in children with whooping cough.
