摘要
《伤寒论》麻子仁丸证,又称"脾约"证,是阳明病重要方证之一。它是阳明病燥热在发展过程中常常出现的一个证候,其病机为阳明燥热损伤太阴阴津,导致脾的转运功能失常而发生的津液偏渗现象,属于阳明病的虚实夹杂证,是阳明太阴同病,而实在阳明,虚在太阴,为实中夹虚。治疗脾约证应把好三环节:泻胃热、滋脾阴、润肠燥。麻子仁丸为阳明、太阴同治之方,方用小承气泻阳明之热,芍药滋太阴之阴,麻仁、杏仁润阳明与滋脾阴并施。并对方证所涉之"脾藏营"理论、厚朴用量、《伤寒论》趺阳诊法的内容及临床意义等进行了解读与研究。
The Syndrome of Maziren Pill in Treatise on Febrile Diseases,also known as“Spleen Yue”syndrome,is one of the important syndromes of Yangming disease.It is a syndrome that often appears in the development of fever of Yangming disease,and its pathogenesis is that Yangming fever damages Taiyin fluid,which leads to abnormal transfer function of spleen and causes the phenomenon of partial body fluid seeps into the bladder,which belongs to the mixed syndrome of deficiency and excess of Yangming disease.It is a symptom that often occurs during the development of dry heat in Yangming disease.Its pathogenesis is that Yangming dry heat damages Taiyin,Yin and fluid,which leads to abnormal spleen transport function.It is the same disease of Yangming and Taiyin,but the reality is Yangming,the deficiency is in the Taiyin,which is the deficiency in the reality.Three steps should be taken in the treatment of spleen syndrome:reducing stomach heat,nourishing spleen yin,and moisturizing intestinal dryness.Maziren pill is a prescription for the treatment of both Yangming and Taiyin at the same time.The prescription uses Xiaochengqi to relieve the heat of Yangming,peony nourishes the Yin of Taiyin,and hemp and apricot nourish Yangming and nourish spleen and yin.The theory of“spleen hiding Yin-nourishing”,the dosage of Magnolia,and the content and clinical significance of Fu-Yang diagnosis in treatise on febrile diseases were studied.
