摘要
目的:研究双源CT对肺动脉CT血管成像图像质量及辐射剂量的影响因素。方法:选取在医院应用双源CT检查的104例疑似肺动脉栓塞(PE)患者,按照随机数表法将其分为观察组和普通单管螺旋组,每组52例。观察组采用双源CT Flash模式进行扫描,普通单管螺旋组采用双源CT常规模式进行扫描。比较两组肺动脉CT血管成像图像质量的主观评分(各级分支评分、边缘评分),并对两组的主干CT值、信号噪声比(SNR)、对比噪声比(CNR)以及辐射剂量[扫描长度、CT容积计量指数(CTDI_(vol))、剂量-长度乘积值(DLP)、有效剂量(ED)进行评价。结果:两组患者的图像各级分支评分、边缘评分和主干CT值、SNR及CNR比较,差异均无统计学意义,两种不同扫描模式的扫描长度比较,差异亦无统计学意义。普通单管螺旋组扫描时间为(1.98±0.58)s,长于观察组(0.98±0.25)s,差异有统计学意义(t=11.417,P<0.05)。普通单管螺旋组TDI_(vol)、DLP及ED高于观察组,差异均有统计学意义(t=2.222,t=5.566,t=5.02;P<0.05)。结论:西门子双源CT的Flash模式扫描方式获得的主观、客观影像质量较好,且可显著降低辐射剂量,减轻检查时机体的辐射损伤。
Objective:To explore the influence of dual-source CT on image quality and radiation dose of computed tomography(CT)angiography of pulmonary artery.Methods:104 patients with suspected pulmonary embolism(PE)who were examined by dual-source CT in hospital were selected as the research objects.According to the random number table method,they were randomly divided into observation group and general single-tube spiral group,with 52 cases in each group.The observation group adopted dual-source CT Flash mode for scanning,and the general singletube spiral group adopted routine mode of dual-source CT for scanning.The subjective scores(branch score of each grade,edge score)of image quality of CT angiography of pulmonary artery between two groups were compared.And the CT value of main branch of blood vessel,signal-to-noise ratio(SNR),contrast-to-noise ratio(CNR),radiation dose[scan length,CT volume measurement index(CTDI_(vol)),dose-length product(DLP)value and effective dose(ED)] of the two groups were further compared.Results:The differences of branch scores of each grade,edge score,CT value of main branch,SNR and CNR between two groups were no significant,and the difference of scan length between two scan modes was not significant.The scan time of general single-tube spiral group was(1.98±0.58)s,which was significantly longer than that[(0.98±0.25)s]of observation group(t=11.417,P<0.05).CTDI_(vol),DLP and ED in single-tube spiral group were significantly higher than those in observation group,and the differences of them between two groups were statistically significant(t=2.222,t=5.566,t=5.02,P<0.05).Conclusion:The Flash mode scanning mode of Siemens dual-source CT can obtain better subjective and objective image quality,and can significantly reduce radiation dose and radiation damage on body during the examination.
