摘要
随着时代的发展,中医骨伤科疾病谱开始出现变化,慢性筋骨病迅速增加,传统的中医筋、骨概念对本类疾病临床治疗的指导性时有不足。林定坤教授基于中医筋骨失衡理念、结合现代解剖学知识,将筋骨概念所涉及的要素按"筋、节、髓、骨"进行区分,分筋伤、节错、髓伤、骨病4个证型进行辨证,在治疗上遵循理筋、调节、护髓、治骨的治则,具体采用内外结合的治法(手法、手术、中药内服等)。运用筋骨辨证分型理念来诊疗慢性筋骨病,有助于阐明其病理机制,提高疗效,为中医骨伤科的现代应用研究和临床实践提供理论基础。
With the development of the times, the disease spectrum of orthopedics and traumatology of traditional Chinese medicine(TCM) begins to change, and the incidence of chronic tendon and bone diseases increases rapidly. As the traditional concepts of tendons and bones in traditional Chinese medicine cannot fully satisfy the needs of clinical application, Professor Lin Dingkun combines the TCM concept of tendon-bone disharmony with modern anatomical knowledge to differentiate the involved tissues into “tendons, joints, marrow and bones”. The chronic tendon and bone diseases are therefore classified into four syndromes, i.e., tendon injury, joint dislocation, marrow injury and bone disease. In terms of treatment, Professor Lin follows the treatment principles of tendon regulation, joint adjustment, marrow protection and bone treatment, and specifically adopts internal and external treatment methods(manipulation, surgery and TCM oral administration, etc.). The application of tendon-bone disharmony theory based syndrome differentiation and classification to the diagnosis and treatment of chronic tendon and bone diseases can facilitate the clarification of its pathological mechanism, improve the therapeutic effect, and provide theoretical basis for modern applied research and clinical practice of TCM orthopedics and traumatology.
