摘要
目的研究外周血及肿瘤组织中膜联蛋白A3、缺氧诱导因子-1α(HIF-1α)表达对结直肠癌铂类耐药的预测价值。方法收集102例结直肠癌铂类药物化疗患者的病历资料,根据患者首个化疗周期对铂类药物的敏感情况分为有效组(n=63)与耐药组(n=39),对比两组患者外周血与肿瘤组织中膜联蛋白A3、HIF-1α表达水平,分析外周血及肿瘤组织中膜联蛋白A3、HIF-1α表达水平及二者联合应用对结直肠癌铂类耐药的预测价值。结果耐药组患者外周血中膜联蛋白A3、HIF-1α表达水平均明显高于有效组,差异均有统计学意义(P﹤0.01)。外周血中膜联蛋白A3与HIF-1α联合应用对结直肠癌铂类耐药具有较高预测价值,膜联蛋白A3最佳截断值为1.970 mg/L,灵敏度为76.92%,特异度为100%;HIF-1α具有中等预测价值,最佳截断值为183.750 pg/ml,灵敏度为76.92%,特异度为68.25%。肿瘤组织中膜联蛋白A3、HIF-1α及二者联合对结直肠癌铂类耐药均具有中等预测价值。结论外周血及肿瘤组织中膜联蛋白A3、HIF-1α表达水平对结直肠癌铂类药物耐药具有中等至较高预测价值,两项指标联合应用时可提高诊断价值,特别是外周血中膜联蛋白A3、HIF-1α表达水平的检测具有取材便捷、重复性好、检测结果量化性等优势。
Objective To invesigate the predictive value of annexin A3 and hypoxia-inducible factor-1α(HIF-1α)ex-pression in peripheral blood and tumor tissues for platinum resistance in colorectal cancer.Method The medical records of 102 patients with colorectal cancer undergoing platinum-based chemotherapy were collected and then divided into the effective group(n=63)and drug-resistant group(n=39)according to their sensitivity to platinum-based drugs in the first chemotherapy cycle.The expression levels of annexin A3 and HIF-1αin the peripheral blood and tumor tissues of the two groups of patients were compared,and their expression levels in the peripheral blood and tumor tissues,as well as the combined application’s predictive value for drug resistance,were also analyzed.Result The expression levels of annex-in A3 and HIF-1αin the peripheral blood of the drug-resistant group were significantly higher than those of the effective group,and the differences were statistically significant(P<0.01).The combined application of annexin A3 and HIF-1αin peripheral blood had a high predictive value for platinum resistance in colorectal cancer.The best cut-off value of annexin A3 was 1.970 mg/L,the sensitivity was 76.92%,and the specificity was 100%;HIF-1αhad a medium predictive value,the best cut-off value was 183.750 pg/ml,the sensitivity was 76.92%,and the specificity was 68.25%.Annexin A3,HIF-1α,and their combination in tumor tissues had moderate predictive value for platinum resistance in colorectal cancer.Conclusion The expression levels of annexin A3 and HIF-1αin peripheral blood and tumor tissues have moderate to high predictive value for platinum drug resistance in colorectal cancer.The combined application of the two indicators can improve the diagnostic value.In particular,the detection of annexin A3 and HIF-1αexpression levels in peripheral blood has the advantages of convenient sampling,good reproducibility,and quantitative detection results.
