体素内不相干运动成像序列联合双参数MRI对前列腺癌的诊断价值 被引量:1

Value of intravoxel incoherent motion sequence combined with dual-parameter MRI in the diagnosis of prostate cancer

摘要 目的探讨体素内不相干运动(IVIM)成像序列联合双参数MRI对前列腺癌的诊断价值。方法将52例良性前列腺增生(BPH)患者设为对照组,54例前列腺癌患者设为观察组,两组患者均行IVIM序列联合双参数MRI检查,比较两组患者的速率常数(K_(ep))、容量转移常数(K^(trans))、血管容量百分数(V_(p))、血管外细胞外间隙容积比(V_(e)),纯扩散系数(D)、伪扩散系数(D^(*))、灌注分数(f),采用受试者工作特征(ROC)曲线进行诊断价值分析,计算ROC曲线下面积(AUC)、灵敏度及特异度。结果观察组患者K^(trans)、Kep值均明显高于对照组,D、D^(*)、V_(e)、f值均明显低于对照组,差异均有统计学意义(P﹤0.01)。高分化前列腺癌患者D^(*)值高于中分化、低分化患者,中分化前列腺癌患者D^(*)值高于低分化患者,差异均有统计学意义(P﹤0.05)。高分化前列腺癌患者f值均低于中分化、低分化患者,中分化前列腺癌患者f值低于低分化患者,差异均有统计学意义(P﹤0.05)。高分化前列腺癌患者K_(ep)值低于中分化患者,而中分化前列腺癌患者K_(ep)值高于低分化患者,差异均有统计学意义(P﹤0.05)。不同分化程度前列腺癌患者D、Ve、K^(trans)值比较,差异均无统计学意义(P﹥0.05)。D、D^(*)、f、K_(ep)、Ve、K^(trans)值诊断前列腺癌的AUC均在0.6以上,且D值最高。结论IVIM联合双参数MRI对前列腺癌的诊断效能较高,对鉴别前列腺癌及前列腺增生有重要参考价值。 Objective To explore the diagnostic value of intravoxel incoherent motion(IVIM)sequence combined with dual-parameter MRI for prostate cancer.Method A total of 52 patients with benign prostatic hyperplasia(BPH)were enrolled as the control group,while 54 patients with prostate cancer were enrolled as the observation group.Both groups underwent IVIM sequence combined with dual-parameter MRI,the rate constant(K_(ep)),volume transfer constant(K^(trans)),percentage of vascular volume(V_(P)),extracellular space volume ratio(V_(e)),pure diffusion coefficient(D),pseudo diffusion coefficient(D^(*)),perfusion fraction(f)were compared between the two groups.The diagnostic value was analyzed by receiver operating characteristic(ROC)curve,and calculated the area under the curve(AUC),sensitivity and specificity.Result The values of K^(trans) and K_(ep) in the observation group were significantly higher than those of the control group,the values of D,D^(*),V_(e),and f in the observation group were significantly lower than those of the control group,the differences were statistically significant(P<0.01).The value of D^(*) in highly differentiated patients were higher than those of moderately differentiated and poorly differentiated patients,and the value of D^(*) in moderately differentiated patients were higher than those of poorly differentiated patients,the differences were statistically significant(P<0.05).The value of f in highly differentiated patients were lower than those of moderately differentiated and poorly differentiated patients,and the value of f in moderately differentiated patients were lower than those of poorly differentiated patients,the differences were statistically significant(P<0.05).The value of K_(ep) in highly differentiated patients were lower than those of moderately differentiated patients,while the value of K_(ep) moderately differentiated patients were higher than those of poorly differentiated patients,the differences were statistically significant(P<0.05).There was no statistically significant difference in the values of D,V_(e) and K^(trans) in patients with differently differentiated prostate cancer(P>0.05).The D,D^(*),f,Kep,Ve and K^(trans) diagnosed prostate cancer with AUC were all above 0.6,and the AUC of D was the highest.Conclusion IVIM combined with dual-parameter MRI has a high diagnostic efficiency for prostate cancer,which has important refer-ence value for differentiating prostate cancer and prostatic hyperplasia.
